【摘要】：目的：結(jié)合臨床相關(guān)病歷統(tǒng)計,對重慶地區(qū)中青年患者顳下頜關(guān)節(jié)紊亂病疼痛的常見誘因進行統(tǒng)計學(xué)分析,為顳下頜關(guān)節(jié)紊亂病疼痛的診斷,治療及預(yù)防提供臨床參考資料。方法：本次研究對象為2013年9月～2014年9月期間在重慶醫(yī)科大學(xué)附屬口腔醫(yī)院就診的重慶地區(qū)患有顳下頜關(guān)節(jié)紊亂病的中青年患者(年齡為20歲-30歲),從中隨機抽取100人,統(tǒng)計其臨床相關(guān)病例和�？茩z查結(jié)果,并記錄患者是否受偏側(cè)咀嚼、夜磨牙、錯頜畸形、精神因素、社會經(jīng)濟學(xué)地位、緊咬牙、大張口、溫度變化、咬硬物等因素影響。對顳下頜關(guān)節(jié)紊亂病疼痛的相關(guān)誘因進行統(tǒng)計分析,采用X2檢驗和二分類的logistic回歸分析,了解各種誘因與顳下頜關(guān)節(jié)紊亂病疼痛之間的相關(guān)性。結(jié)果：本研究表明,偏側(cè)咀嚼(P0.001),夜磨牙(P=0.009),精神因素(P=0.001)與顳下頜關(guān)節(jié)紊亂病疼痛密切相關(guān)。顳下頜關(guān)節(jié)紊亂病疼痛與偏側(cè)咀嚼的相關(guān)性最大(OR=6.334, 95%CI=2.284-17.565),其次為精神因素(OR=4.330,95%CI=1.232.15.216),最后為夜磨牙(OR=3.370,95％CI=1.041.10.907)。在有偏側(cè)咀嚼習(xí)慣的患者中,關(guān)節(jié)疼痛(P=0.040)或肌筋膜疼痛(P=0.044)癥狀的出現(xiàn)均與偏側(cè)咀嚼有關(guān),而且非咀嚼慣用側(cè)關(guān)節(jié)疼痛和肌筋膜疼痛的患者較多,具有統(tǒng)計學(xué)意義。結(jié)論：重慶地區(qū)中青年患者顳下頜關(guān)節(jié)紊亂病疼痛的主要誘因為偏側(cè)咀嚼習(xí)慣、夜磨牙和精神因素。顳下頜關(guān)節(jié)紊亂病的各項因素相互作用,目前尚不清楚各因素與顳下頜關(guān)節(jié)紊亂病疼痛的因果關(guān)系。
[Abstract]:Objective: to analyze the common causes of temporomandibular joint disorder (TMJ) pain in young and middle-aged patients in Chongqing, and to make a diagnosis of temporomandibular joint disorder (TMJ) pain. Methods: from September 2013 to September 2014, the subjects of this study were young and middle-aged patients with temporomandibular joint disorders in Chongqing from September 2013 to September 2014. (between 20 and 30 years of age, 100 people were randomly selected from the middle. The clinical related cases and the results of special examinations were counted, and whether the patients suffered from unilateral mastication, night molars, malocclusion, mental factors, social economic status, clenching teeth, large mouth opening, temperature change were recorded. Statistical analysis of the related inducement of temporomandibular joint disorder pain, using X2 test and logistic regression analysis of two classification, To understand the correlation between various triggers and pain in temporomandibular disorders. Results: this study shows that. The relationship between the pain of temporomandibular joint disorder and the pain of temporomandibular joint disorder is the highest: OR6.334,95CII 2.284-17.565, followed by the mental factor OR4.33095CII 1.232.15.216, and finally the or37095CII 1.041.10.9070.The mental factor OR4.33095CII 1.232.15.216and finally the or3.37095CII 1.041.10.907 are found to be closely related to the pain of temporomandibular joint disorder (temporomandibular joint disorder), and the relationship between the pain of temporomandibular joint disorder and unilateral mastication is the highest (OR6.334,95284-17.565). Among patients with unilateral chewing habits, The occurrence of joint pain (P0. 040) or myofascial pain (P0. 044) was associated with unilateral mastication, and there were more patients with non masticatory side joint pain and myofascial pain. Conclusion: the main causes of temporomandibular joint disorder pain in young and middle-aged patients in Chongqing are unilateral mastication habits, night molars and mental factors, and the interaction of various factors of temporomandibular joint disorder. The causal relationship between the factors and temporomandibular joint disorder pain is unclear.
