摻鍶透鈣磷石骨水泥修復(fù)牙槽骨缺損的實(shí)驗(yàn)研究

【摘要】:目的通過材料的性能檢測、生物活性判斷、影像學(xué)檢查來評價(jià)摻鍶透鈣磷石骨水泥修復(fù)家兔牙槽骨缺損的能力,觀察堿性成纖維細(xì)胞生長因子(Basic Fibroblast Growth Factor,b-FGF)和骨保護(hù)素(Osteoprotegerin,OPG)在骨缺損區(qū)的表達(dá)和分布情況,探討摻鍶透鈣磷石骨水泥的作用機(jī)制,研究摻鍶透鈣磷石骨水泥作為骨替代材料的可行性,為臨床應(yīng)用提供理論依據(jù)。方法制備透鈣磷石,并按Sr/(Sr+Ca)為5%和10%加入氯化鍶,制備摻鍶透鈣磷石骨水泥。通過X線衍射儀(X-ray diffraction,XRD)、掃描電鏡(Scanning electron microscope,SEM)、萬能材料實(shí)驗(yàn)機(jī)等對材料的物相、表面形態(tài)和抗壓強(qiáng)度進(jìn)行測量和分析。采用急性溶血性實(shí)驗(yàn)和皮膚刺激反應(yīng)試驗(yàn)檢測材料的安全性及生物相容性。健康家兔36只隨機(jī)分成四組,分別為空白對照組、透鈣磷石組、摻鍶5%透鈣磷石組和摻鍶10%透鈣磷石組,每組動(dòng)物9只。所有動(dòng)物均建立雙側(cè)下頜牙槽骨門齒遠(yuǎn)端無牙區(qū)10×3×4mm骨缺損,空白組缺損處不充填任何材料,其余實(shí)驗(yàn)各組分別充填相對應(yīng)的骨水泥塊。術(shù)后分別于4,8,12周處死每組動(dòng)物各3只,通過CBCT觀察影像學(xué)變化,分析觀察缺損區(qū)b-FGF、OPG兩種因子的表達(dá)與分布情況,利用Image-Pro Plus6.0軟件測量同一倍數(shù)下b-FGF、OPG陽性表達(dá)部位的平均光密度值(MOD值),各組間進(jìn)行比較,統(tǒng)計(jì)學(xué)處理。結(jié)果1成功制備摻鍶透鈣磷石骨水泥,衍射結(jié)果分析鍶以磷酸氫鍶的形式存在于骨水泥中;電鏡圖片顯示樣品由大量短棒狀、片狀、團(tuán)簇狀的晶體結(jié)構(gòu)組成,結(jié)構(gòu)間有大小不等的微孔;鍶的摻入降低了透鈣磷石的抗壓強(qiáng)度,且與摻入鍶的濃度相關(guān),即摻鍶10%透鈣磷石的強(qiáng)度低于摻鍶5%透鈣磷石強(qiáng)度。2生物相容性實(shí)驗(yàn)結(jié)果示三組骨水泥溶血率均低于國家要求標(biāo)準(zhǔn),體外試驗(yàn)和體內(nèi)試驗(yàn)均證明符合材料安全,無毒。3 4周時(shí)摻鍶5%和10%組骨水泥開始溶解、吸收;透鈣磷石組骨水泥未溶解;空白組缺損明顯,無新生骨組織。8周時(shí)摻鍶透鈣磷石的兩組缺損已基本修復(fù),修復(fù)早于未摻鍶組,且摻鍶10%透鈣磷石組較摻鍶5%透鈣磷石組修復(fù)效果好,新生骨面光滑平整;透鈣磷石組缺損區(qū)稍凹于周圍骨組織,新生骨表面不光滑;空白組缺損區(qū)為凹坑狀,無新生骨組織。12周時(shí)空白組未完成缺損的修復(fù);透鈣磷石組缺損區(qū)基本修復(fù),形成骨小梁等結(jié)構(gòu);摻鍶組修復(fù)完成。4免疫組化結(jié)果分析:組內(nèi)比較時(shí),b-FGF和OPG的陽性表達(dá)都隨著時(shí)間的延長呈下降趨勢。b-FGF的表達(dá),4周時(shí),各組間比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),其中空白組MOD值最小,摻鍶10%透鈣磷石組MOD值最大;8周時(shí),各組間比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05),摻鍶5%透鈣磷石組MOD值最大,摻鍶組高于透鈣磷石組,空白組MOD值最小;12周時(shí)四組差異無統(tǒng)計(jì)學(xué)意義(P0.05)。OPG在4周和8周時(shí)的MOD值均為摻鍶5%透鈣磷石組最高,摻鍶10%透鈣磷石組高于透鈣磷石組,空白對照組最小,各組之間差異有統(tǒng)計(jì)學(xué)意義(P0.05);12周時(shí),空白組和透鈣磷石組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05),摻鍶5%透鈣磷石組和摻鍶10%組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05),后兩組和前兩組交叉比較時(shí)差異有統(tǒng)計(jì)學(xué)意義(P0.05),后兩組MOD值較高。析因分析結(jié)果顯示,MOD值的變化既與骨水泥的作用時(shí)間直接相關(guān),又與組別差異相關(guān),同時(shí)二者之間具有交互作用。鍶的摻入可以使b-FGF和OPG的表達(dá)增加,促進(jìn)成骨。結(jié)論1本實(shí)驗(yàn)所采用骨水泥的制備方法可靠,能簡單復(fù)制,且材料的抗壓強(qiáng)度與松質(zhì)骨強(qiáng)度相近。2透鈣磷石、摻鍶5%、10%透鈣磷石骨水泥基本無毒性,為較安全的植入材料。3摻鍶透鈣磷石骨水泥能夠在修復(fù)早期促進(jìn)b-FGF和OPG的增加,促進(jìn)成骨過程。4摻鍶5%和摻鍶10%透鈣磷石骨水泥能縮短缺損的修復(fù)時(shí)間,且摻鍶量為10%時(shí),較摻鍶5%透鈣磷石修復(fù)效果明顯。
