替牙期單側(cè)完全性唇腭裂患者頜面部對稱性研究

【摘要】:目的:對替牙期單側(cè)完全性唇腭裂(Unilateral complete cleft lip and palate, UCLP)患者與非唇腭裂患者的頭顱定位正位片、全頜曲面斷層片進行對比分析,研究替牙期UCLP患者頜面部垂直向與橫向的對稱性,并進一步探討UCLP患者上下頜骨發(fā)育不對稱間的相關性。 方法:選擇30例替牙期UCLP患者(男16例,女14例)作為UCLP組,30例替牙期非唇腭裂患者(男15例,女15例)作為對照組。應用人工描繪定點,通過頭顱定位正位片與全頜曲面斷層片對兩組患者的頜面部硬組織相關測量項目進行比較,分析UCLP患者頜面部垂直向與橫向的對稱性。所有數(shù)據(jù)使用SPSS19.0統(tǒng)計軟件進行統(tǒng)計分析。 結(jié)果: 1.垂直向?qū)ΨQ性: ①垂直向,替牙期UCLP患者兩側(cè)MxH、NcH、NaH、CoH、GoH、AgH、CH。RH、 CH+RH、d-Ag均存在不對稱,差異具有統(tǒng)計學意義(P0.05);其中非裂隙側(cè)的CoH、GoH、 AgH、CH、RH、CH+RH大于裂隙側(cè)。 ②替牙期UCLP患者頜面部垂直向非對稱率較大的部位在Na點(8.76%)、Nc點(7.8%)。 ③與對照組相比,替牙期UCLP患者在Mx、Nc、Na、Co、Go、Ag、CH、RH、CH+RH、 d-Ag上的垂直向非對稱率有差異且有統(tǒng)計學意義(P0.05)。 ④在垂直向,UCLP患者Go的非對稱率與Nc的非對稱率,存在一定程度地相關性(P0.05)。 2.橫向?qū)ΨQ性: ①橫向,替牙期UCLP患者兩側(cè)MxV、NcV、NaV、CoV、GoV、AgV均存在不對稱,且差異具有統(tǒng)計學意義(P0.05),且非裂隙側(cè)上下頜骨的橫向?qū)挾却笥诹严秱?cè)。 ②替牙期UCLP患者頜面部橫向非對稱率較大的部位在Na點(29.19%)、Nc點(21.99%)。 ③橫向,替牙期UCLP患者Ag的非對稱率與Nc的非對稱率存在相關性,且有統(tǒng)計學意義(P0.05)。 結(jié)論: ①替牙期UCLP患者頜面部存在垂直向與橫向的不對稱畸形。 ②替牙期UCLP患者的上頜骨不對稱畸形比下頜骨嚴重,其中以梨狀孔區(qū)最為顯著。 ③在垂直向與橫向,替牙期UCLP患者裂隙側(cè)下頜骨較健側(cè)發(fā)育不足;在橫向,替牙期UCLP患者裂隙側(cè)上頜骨較健側(cè)發(fā)育不足。 ④替牙期UCLP患者上下頜骨的非對稱率有一定的相關性。
[Abstract]:Objective: to study the vertical and transverse symmetry of maxillofacial region in patients with unilateral complete cleft lip and palate (UCLP) and non-cleft cleft lip and palate (UCLP) patients with unilateral unilateral cleft lip and palate (UCLP) and non-cleft lip and palate (non-cleft lip and palate) patients with unilateral unilateral cleft lip and palate. And to further explore the relationship between the development asymmetry of the upper and lower jaw in UCLP patients. Methods: 30 UCLP patients (16 males and 14 females) were selected as control group in UCLP group, 30 patients with cleft lip and palate (15 males and 15 females) were selected as control group. The vertical and lateral symmetry of maxillofacial hard tissue in patients with UCLP were analyzed by comparing the items of maxillofacial hard tissue measurement in two groups by cephalometric orthograph and total maxillary surface tomograph. All the data were statistically analyzed by SPSS19.0 statistical software. Results:. 1. Vertical symmetry:. 1 in the vertical direction, there was asymmetry in both sides of UCLP patients with UCLP. The difference was statistically significant (P0.05A), and the RHCH in the non-fractured side was larger than that in the fissured side (P < 0.05), and the RHCH RH in the unfractured side was higher than that in the fissure side (P < 0.05). The results were as follows: (1) there was no symmetry in both sides of the patients with UCLP in the orthodontic phase, and the difference was statistically significant (P0.05A). (2) the higher vertical asymmetry of maxillofacial region in UCLP patients during dental replacement stage was found at Na point 8.76% and NC point 7.8%. 3Compared with the control group, there was a significant difference in the vertical asymmetry rate of the UCLP patients in the Mxncncncncncna CoCoA, CHH, d-Ag in the RHH, d-Ag, and there was significant difference in the vertical asymmetry rate between the patients and the control group (P 0.05). (4) there was a certain correlation between the asymmetric rate of go and the asymmetric rate of NC in the patients with UCLP. 2. Horizontal symmetry:. 1in transverse, both sides of UCLP patients with UCLP had asymmetry in both sides, and the difference was statistically significant (P 0.05), and the transverse width of the upper and lower bones in the non-fissured side was larger than that in the fissured side. 2the greater transverse asymmetry rate of maxillofacial region in patients with UCLP in dental replacement stage was found at Na point 29.19 and NC point 21.99%. 3Transverse, the asymmetrical rate of Ag was correlated with the asymmetrical rate of NC in patients with UCLP in dental replacement stage, and there was statistical significance (P 0.05). Conclusion:. 1 there were vertical and transverse asymmetry in maxillofacial region in patients with UCLP at dental replacement stage. 2the asymmetry of maxilla in UCLP patients was more serious than that in mandible, especially in piriform foramen. (3) in vertical and lateral directions, the mandible of fissure side of UCLP patients was less developed than that of healthy side, and the maxillary bone of fissured side of patients with UCLP was underdeveloped in transverse direction. 4 there was a certain correlation between the asymmetry rate of upper and mandibular bone in UCLP patients.
