【摘要】：目的:研究上頜第一磨牙近中舌尖缺損同時(shí)伴有不同深度隱裂紋及不同修復(fù)方式修復(fù)后其應(yīng)力分布以及裂紋擴(kuò)展情況。方法:利用錐形束CT及有限元軟件,建立上頜第一磨牙近中舌尖缺損伴缺損斷面有不同深度隱裂紋的有限元模型,對模型分別采用不同修復(fù)方式進(jìn)行修復(fù),并模擬在不同咬合力下的受力情況,觀察分析裂紋的等效應(yīng)力值及應(yīng)力分布、裂紋J積分值、牙本質(zhì)最大主應(yīng)力值。結(jié)果:采用金屬樁加全瓷冠修復(fù)3種深度隱裂牙時(shí),裂紋的等效應(yīng)力值、J積分值、牙本質(zhì)最大主應(yīng)力值均最小。裂紋深度增加時(shí)牙齒垂直向和斜向受力時(shí)的裂紋等效應(yīng)力值、J積分值、牙本質(zhì)最大主應(yīng)力值均明顯增大。每一種修復(fù)方式中4種受力方向時(shí)裂紋等效應(yīng)力值、J積分值、牙本質(zhì)最大主應(yīng)力值均為為200 N、90°200 N、45°600 N、0°200 N。結(jié)論:采用彈性模量大的金屬樁加全冠修復(fù)時(shí),隱裂紋擴(kuò)展及牙折的可能性較小;隱裂紋越深修復(fù)后隱裂紋擴(kuò)展及牙折的可能性越大;側(cè)向力比垂直向力對裂紋及牙本質(zhì)的應(yīng)力變化影響大且易造成裂紋擴(kuò)展和牙折。
[Abstract]:Objective: to study the stress distribution and crack propagation of maxillary first molars with different depth of hidden cracks and different repair methods. Methods: Cone-beam CT and finite element software were used to study the stress distribution and crack propagation of maxillary first molars. The finite element model of the maxillary first molars with defects at the proximal and middle tongue tip of the maxillary first molar with different depth hidden cracks was established. The models were repaired with different repair methods, and the stress under different occlusal forces was simulated. The equal effect force value and stress distribution of crack, J-integral value of crack, maximum principal stress value of dentin were observed and analyzed. Results: when three kinds of deep cracked teeth were repaired with metal post and all-ceramic crown, the equal effect force value of crack was equal to J integral value. The maximum principal stress of dentin is the smallest. When the depth of crack increases, the equal effect force value of crack and J integral value of tooth under vertical and oblique force are obtained. The maximum principal stress of dentin increased obviously. The maximum principal stress values of dentin are all 200 Ne 90 擄200 N, 45 擄600 N and 0 擄200N.Conclusion: when the metal pile with high elastic modulus is repaired with full crown, the probability of hidden crack propagation and tooth fracture is smaller, the deeper the hidden crack is, the greater the probability of the growth of hidden crack and tooth fracture is. The lateral force ratio has a great influence on the stress change of the crack and dentin, and it is easy to cause crack propagation and tooth fracture.
