口內(nèi)掃描儀全牙列掃描精度及不同操作者間掃描精度差異的研究

【摘要】:目的:本研究以硅橡膠印模灌制的石膏模型經(jīng)模型掃描儀掃描所得數(shù)據(jù)作為參考,評估口內(nèi)掃描儀(iTero(?)ElementTM,Align Tech.,USA)分別于口內(nèi)掃描天然牙列的精度(accuracy)以及于體外掃描石膏模型的精度。另外通過不同操作者使用同一臺掃描儀對放置于仿真頭模環(huán)境中及口外直視環(huán)境下的同一標(biāo)準(zhǔn)模型進(jìn)行掃描,評估不同操作者間結(jié)果的差異性。材料和方法:1.口內(nèi)掃描儀掃描全牙列精度的研究選取符合條件的志愿者15名,使用硅橡膠(Polyvinyl Siloxane,PVS)印模材料制取印模并灌制石膏模型,使用模型掃描儀(Activity888,Smart optic,Germany)獲取石膏數(shù)字化模型(P組)作為參考模型,使用iTero Element分別于口內(nèi)掃描天然牙列獲取掃描模型(T組)、于體外掃描相應(yīng)石膏模型獲取數(shù)字模型(D組)作為測試模型,用以評價掃描的準(zhǔn)確度(trueness)。選取志愿者一名,使用iTero分別于口內(nèi)掃描全牙列獲取數(shù)字模型10副、于口外掃描石膏模型獲取數(shù)字模型10副,用以評價掃描的精密度(precision)。2.不同操作者間掃描精度差異性的研究制作與仿真頭模相匹配的一副標(biāo)準(zhǔn)石膏模型作為掃描對象,由10名正畸醫(yī)師分別于仿真頭模環(huán)境下、口外直視環(huán)境下使用同一臺iTero掃描儀對該標(biāo)準(zhǔn)模型掃描,獲取數(shù)字模型,并使用模型掃描儀獲取參考數(shù)字模型(R組)。3.數(shù)據(jù)處理將以上模型的.stl文件導(dǎo)入3D圖像校核軟件中,進(jìn)行邊緣修整,各組間、組內(nèi)將相對應(yīng)的參考模型、測試模型進(jìn)行配準(zhǔn),使用3D比較功能比較分析兩模型間的偏離程度并記錄數(shù)據(jù),其中較小的RMS值提示兩模型間較好的一致性。將各組差異值導(dǎo)入 SPSS(IBM SPSS Statistics 21,IBM,NY,USA),以上下頜及不同環(huán)境進(jìn)行分組。使用Kolmogorov-Smirnoff檢驗各組數(shù)值的正態(tài)分布,使用Levene's test檢驗方差齊性。如符合正態(tài)分布及方差齊性,則使用t檢驗比較兩組間差異性;如不符合則使用Kruskal-Wallis檢驗。若P0.05提示有顯著性差異。結(jié)果:1.口內(nèi)掃描儀掃描全牙列精度的研究口內(nèi)掃描天然牙列的準(zhǔn)確度為99.99±26.80μm,體外掃描石膏模型的準(zhǔn)確度為80.74±20.33μm。比較兩種環(huán)境下準(zhǔn)確度差異性,P0.01,具有顯著差異性。口內(nèi)掃描天然牙列的準(zhǔn)確度的精密度為77.05±12.73μm,體外掃描石膏模型的精密度為46.72±16.35μm,比較兩者差異性,P0.01,具有顯著差異性。2.不同操作者間掃描精度差異性的研究在仿真頭模環(huán)境下,全口掃描準(zhǔn)確度為104.80±13.41μm,在口外直視環(huán)境下,全口掃描準(zhǔn)確度為106.36士12.37μm。對比兩種環(huán)境下模型掃描的準(zhǔn)確度,P0.05,無顯著差異。上下頜模型掃描準(zhǔn)確度間存在顯著差異。在仿真頭模環(huán)境下,全口掃描精密度為49.40±19.10μm;在口外直視環(huán)境下,全口掃描精密度為59.54±19.43μm,下頜掃描精密度優(yōu)于上頜掃描精密度。對比兩種環(huán)境下掃描的精密度,P0.05,具有顯著差異性。結(jié)論1.以模型掃描儀數(shù)據(jù)為參考,iTero Element掃描全牙列精度在正畸臨床可接受范圍內(nèi),但體外掃描石膏模型精度優(yōu)于口內(nèi)掃描天然牙列精度。2.不同操作者使用iTero Element掃描精度存在一定程度的差異性,但對于正畸臨床工作影響較小。
[Abstract]:Objective: To study the plaster model of silicone rubber impression made by the scanner as a reference data model, evaluation of intraoral scanners (iTero (?) ElementTM, Align Tech., USA) scan the natural dentition in precision respectively (accuracy) and precision in vitro scanning plaster model. In addition by different operators with a scanner to scan placed on the head simulator environment and extraoral open environment with a standard model, the difference between the results of the evaluation of different operators. Materials and methods: 1. intraoral scanning the whole dentition precision study selected in accordance with the conditions of the 15 volunteers, the use of silicone rubber (Polyvinyl Siloxane.
