【摘要】：目的：觀察連續(xù)波熱牙膠充填技術(shù)對(duì)側(cè)支根管充填的療效。 方法：收集在吉林大學(xué)口腔醫(yī)院牙體牙髓病科內(nèi)需要根管治療術(shù)的30例的病例。患牙30顆，年齡18-60歲，男15例，女15例；確認(rèn)無(wú)明顯的瘺管和根尖發(fā)育完成，無(wú)根尖吸收及根折之后進(jìn)行實(shí)驗(yàn)。開(kāi)髓之前用橡皮障隔水，然后臨床常規(guī)的方法開(kāi)髓及成形洞型，并使用拔髓針進(jìn)行拔髓，揭髓室頂及沖洗使術(shù)者視野清晰。探查根管口位置及其數(shù)目，使用10#C型銼小幅度提拉疏通根管，使用根管長(zhǎng)度測(cè)量?jī)x器與X線片結(jié)合距根尖1mm左右處為確定工作長(zhǎng)度。使用乙二胺四乙酸（EDTA）與PROTAPER鎳鈦器結(jié)合預(yù)備根管至F2，預(yù)備過(guò)程中用０.9%生理鹽水和1%次氯酸鈉交替反復(fù)沖洗，預(yù)備后使用超聲蕩洗儀清理根管內(nèi)的玷污層及感染物；隔濕及干燥后，，使用樟苯酚棉捻封藥，并使用暫封材料暫封窩洞，一周后，去除暫封材料后將主牙膠尖蘸一小層根管封閉劑，按工作長(zhǎng)度緩慢插入根管，使用電攜熱器切除距根尖3-4mm處左右的牙膠尖，再加使用熱牙膠注射儀注射于根中上段，同時(shí)使用垂直加壓器嚴(yán)密加壓封閉根管。根管充填完成后，觀察X片上的根管及側(cè)支根管充填的效果。 結(jié)果：在根管治療30個(gè)病例中，用連續(xù)波熱牙膠垂直充填技術(shù)充填的X線片全都顯示良好的封閉效果，其中6例均可發(fā)現(xiàn)側(cè)支根管。 結(jié)論：Protaper鎳鈦器械有利于械形成良好連續(xù)的錐度，根管沖洗的方式和根管沖洗劑的選擇對(duì)側(cè)支根管有影響，連續(xù)波熱牙膠垂直充填技術(shù)對(duì)側(cè)支根管有充填效果良好的封閉性。
[Abstract]:Objective: to observe the effect of continuous wave hot gum filling on lateral branch root canal filling. Methods: a total of 30 patients with dental pulp disease (Kone), aged 18-60 years, 15 males and 15 females, who needed root canal therapy, were collected in Jilin University Stomatology Hospital. It was confirmed that there was no obvious fistula and apical development, no apical absorption and root fracture were carried out. Before pulp opening, rubber barrier was used to isolate water, then clinical routine methods were used to open and shape the cavity, and pulpooning needle was used to pull out the pulp. To explore the location and number of root canal orifice, 10 #C file was used to dredge the root canal. The length of root canal was determined by measuring the length of root canal with X-ray film and about 1 mm from the root tip. The root canal to F2 was prepared by the combination of ethylenediamine tetraacetic acid (EDTA) and PROTAPER nickel titanium apparatus. During preparation, 0.9% normal saline and 1% sodium hypochlorite were alternately rinsed repeatedly. After preparation, the smear layer and infection in root canal were cleaned by ultrasonic washing instrument. After wetting and drying, anisodonol cotton was used to seal the root canal, and the temporary sealing material was used to seal the hole. After a week, the main gum tip was dipped in a small layer of root canal sealant and inserted into the root canal slowly according to the working length. The electric calorimeter was used to remove the gum tip about 3 to 4 mm from the root tip, and then the hot dental glue injector was used to inject the root canal into the upper and middle part of the root. At the same time, the root canal was tightly pressurized and sealed by using the vertical pressurizer. After the root canal filling was completed, the root canal filling was completed. To observe the effect of root canal and lateral branch root canal filling on X-ray. Results: in 30 cases of root canal therapy, all the X-ray films filled with continuous wave hot gum vertical filling showed good sealing effect, of which 6 cases could find lateral branch root canal. Conclusion the ratio Protaper Ni-Ti instrument is beneficial to the formation of a good continuous taper. The way of root canal washing and the choice of root canal washing agent have an effect on the lateral branch root canal. Continuous wave hot gum vertical filling has good sealing effect on lateral branch root canal.
