【摘要】：目的:比較手術(shù)治療的肺部良性腫瘤與早期、中期肺癌患者的焦慮及抑郁癥狀的檢出比率,探討其影響因素。方法:研究對(duì)象為就診于某三級(jí)綜合醫(yī)院的283例接受手術(shù)的肺部腫瘤患者,其中肺部良性腫瘤或病變患者107例,早期肺癌患者58例,中期肺癌患者118例。采用自編調(diào)查表收集社會(huì)人口學(xué)及臨床相關(guān)信息,使用抑郁自評(píng)量表(SDS)和焦慮自評(píng)量表(SAS)分別評(píng)價(jià)腫瘤患者抑郁和焦慮癥狀,SDS標(biāo)準(zhǔn)分53分記為有抑郁癥狀,SAS標(biāo)準(zhǔn)分50分記為有焦慮癥狀。結(jié)果:肺部良性腫瘤或病變患者無明顯焦慮及抑郁癥狀,早期和中期肺癌患者中53.40%存在焦慮癥狀,43.44%存在抑郁癥狀,差異有統(tǒng)計(jì)學(xué)意義(P0.001)。多重線性回歸分析顯示,中期肺癌(β=29.70,P0.001)及年齡65歲以上(β=2.58,P=0.018)是發(fā)生抑郁癥狀的危險(xiǎn)因素,相對(duì)于良性腫瘤及早期肺癌患者,中期肺癌患者焦慮癥狀更重(β=27.22,P0.001)。結(jié)論:中期肺癌較良性腫瘤及早期肺癌更加容易導(dǎo)致患者出現(xiàn)焦慮和抑郁等負(fù)面心理應(yīng)激;且老年腫瘤患者更容易出現(xiàn)抑郁癥狀。
[Abstract]:Objective: to compare the detection rate of anxiety and depression between benign lung tumors treated by operation and patients with early and middle stage lung cancer, and to explore the influencing factors. Methods: 283 patients with pulmonary tumors, including 107 patients with benign pulmonary tumors or lesions, 58 patients with early lung cancer and 118 patients with middle stage lung cancer, were enrolled in the study. The social demography and clinical information were collected by self-designed questionnaire. The depression and anxiety symptoms of tumor patients were evaluated by self-rating depression scale (SDS) and self-rating anxiety scale (SAS). The standard score of SDS was 53 and the score of SAS was 50. Results: there were no obvious anxiety and depression symptoms in patients with benign pulmonary tumors or lesions. 53.40% of patients with early and middle stage lung cancer had anxiety symptoms and 43.44% had depressive symptoms, the difference was statistically significant (P0.001).
