【摘要】：目的觀察烏靈膠囊對卒中后抑郁(post-stroke depression,PSD)的一級預防作用。方法將急性腦卒中患者,采用隨機化方法分兩組。兩組在心腦血管病治療相同基礎(chǔ)上,試驗組(55例)加服烏靈膠囊(每粒0.33 g)每次3粒,1日3次;對照組55例,加服安慰劑每次3粒,1日3次;2周為1個療程。每2周進行PSD診斷,符合PSD停服藥物;不符合PSD繼續(xù)服藥;持續(xù)服藥1~12個療程(共計6個月),若仍不符合PSD,則停止服藥。觀察PSD發(fā)病率、出現(xiàn)抑郁的時間、漢密爾頓抑郁量表(HAMD)評分及不良反應(yīng)。結(jié)果試驗組PSD發(fā)病率1、3、6個月時分別為8%、16%和34%,對照組分別為19.6%、29.4%和54.9%,試驗組發(fā)生率均低于對照組,且6個月時比較,差異有統(tǒng)計學意義(χ2=4.465,P0.05)。試驗組發(fā)生PSD平均時間為(14.96±8.31)周,對照組為(9.36±6.06)周,兩組比較,差異有統(tǒng)計學意義(t=6.762,P0.05);隨訪6個月,試驗組發(fā)生PSD時的HAMD評分為(11.96±2.14)分,對照組為(14.57±4.24)分,兩組比較,差異有統(tǒng)計學意義(t=5.641,P0.05)。結(jié)論烏靈膠囊在降低PSD發(fā)病率、延緩PSD發(fā)生時間、減輕PSD發(fā)生抑郁程度方面均優(yōu)于安慰劑組,對PSD有一定的預防作用。
[Abstract]:Objective to observe the primary preventive effect of Wuling capsule on post-stroke depression (post-stroke depression,PSD). Methods the patients with acute stroke were randomly divided into two groups. On the basis of the same treatment of cardiovascular and cerebrovascular diseases, the experimental group (55 cases) was treated with Wuling capsule (0.33 g each) 3 times a day, while the control group (55 cases) was treated with placebo 3 tablets at a time, 3 times a day, and 2 weeks as a course of treatment. PSD diagnosis was performed every 2 weeks, which was in accordance with PSD withdrawal, non-compliance with PSD and 12 courses of treatment (total 6 months). If not in accordance with PSD, the drug was stopped. The incidence of PSD, the time of depression, Hamilton depression scale (HAMD) score and adverse reactions were observed. Results the incidence of PSD in the experimental group was 1, 3, 8%, 16% and 34% at 6 months, and 19.6%, 29.4% and 54.9% in the control group, respectively. the incidence rate in the experimental group was lower than that in the control group, and the difference was statistically significant at 6 months (蠂 2 鈮，
