【摘要】：雙相障礙是精神科的常見疾病,是一個(gè)重要的全球衛(wèi)生問(wèn)題,情感氣質(zhì)是人們對(duì)環(huán)境刺激作出的反應(yīng),主要包括個(gè)體的生物節(jié)律、情緒、認(rèn)知等[1],國(guó)外研究提示一些情感氣質(zhì)是情感障礙的亞臨床表現(xiàn),可能是情感障礙臨床表現(xiàn)與生物遺傳學(xué)聯(lián)系的紐帶[2]。目前國(guó)內(nèi)雙相障礙情感氣質(zhì)特征的研究較少,未見此方面的系統(tǒng)研究,本研究分為兩部分:第一部分雙相障礙情感氣質(zhì)特征以及雙相Ⅰ型和Ⅱ型障礙情感氣質(zhì)比較目的:探討雙相障礙患者情感氣質(zhì)的特征,分析雙相Ⅰ型障礙和雙相Ⅱ型障礙情感氣質(zhì)特征的差異性。方法:采用橫斷面研究,共納入160例雙相障礙組(雙相Ⅰ型組90例、雙相Ⅱ型組70例)和對(duì)照組150例。采用情感氣質(zhì)評(píng)定量表簡(jiǎn)版中文版評(píng)估雙相障礙患者及對(duì)照組間情感氣質(zhì)類型,比較不同分型情感氣質(zhì)特征的差異性。結(jié)果:雙相障礙組情感氣質(zhì)評(píng)定量表總分及5個(gè)因子分均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(均P0.05),在情感旺盛氣質(zhì)因子得分方面雙相Ⅰ型障礙患者高于雙相Ⅱ型障礙;雙相Ⅰ型障礙患者的抑郁氣質(zhì)因子得分低于雙相Ⅱ型障礙,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:雙相障礙患者存在特殊的情感氣質(zhì),不同亞型之間情感氣質(zhì)類型有差異;雙相Ⅰ型障礙患者具有較高的情感旺盛氣質(zhì),而雙相Ⅱ型障礙患者具有相對(duì)較高的抑郁氣質(zhì)特征。第二部分雙相障礙患者與健康一級(jí)親屬情感氣質(zhì)特征比較目的:探討雙相障礙患者與健康一級(jí)親屬情感氣質(zhì)的特征,分析兩者之間的差異性。方法:本研究共納入75例雙相障礙患者、健康一級(jí)親屬75例和對(duì)照組9 0例。采用情感氣質(zhì)評(píng)定量表簡(jiǎn)版中文版評(píng)估雙相障礙患者及健康一級(jí)親屬之間情感氣質(zhì)類型,并對(duì)情感氣質(zhì)特征的差異性進(jìn)行比較。結(jié)果:在循環(huán)氣質(zhì)因子方面,雙相障礙組評(píng)分高于健康一級(jí)親屬組高于對(duì)照組;在抑郁氣質(zhì)因子及焦慮氣質(zhì)因子方面,雙相障礙組評(píng)分均高于健康一級(jí)親屬組和對(duì)照組;在易激惹氣質(zhì)因子及情感旺盛氣質(zhì)因子方面,對(duì)照組評(píng)分分別低于雙相障礙組和健康一級(jí)親屬組,差異均有統(tǒng)計(jì)學(xué)意義(均P0.05)。結(jié)論:雙相障礙患者的健康一級(jí)親屬同樣存在特殊的情感氣質(zhì),循環(huán)氣質(zhì)最可能是雙相障礙“內(nèi)表型”指標(biāo)之一。
