【摘要】：目的探索注意缺陷多動(dòng)障礙(attention deficit hyperactivity disorder,ADHD)患兒多巴胺轉(zhuǎn)運(yùn)體基因(dopamine transporter gene,DAT1)、多巴胺D4受體基因(dopamine receptor D4 gene,DRD4)啟動(dòng)子區(qū)CpG島甲基化狀態(tài)與其臨床癥狀嚴(yán)重程度的相關(guān)性。方法收集111例ADHD患兒精神類疾病家族史資料,并采用ADHD癥狀分級(jí)父母評(píng)定量表(attention deficit hyperactivity disorder rating scale-Ⅳhome version,ADHD-RS-Ⅳ)及自編對(duì)立違抗障礙(oppositional defiant disorder,ODD)量表對(duì)患者臨床癥狀進(jìn)行評(píng)定。采用亞硫酸氫鈉測(cè)序法,檢測(cè)患者外周血的DAT1、DRD4啟動(dòng)子區(qū)CpG島目標(biāo)片段甲基化狀態(tài)。結(jié)果無抑郁、焦慮和ADHD家族史的患兒DAT1啟動(dòng)子區(qū)甲基化水平高于有抑郁、焦慮或ADHD家族史的患兒(P0.05)。DAT1和DRD4甲基化水平在ADHD-RS-Ⅳ總分(≤30分組與30分組)、注意缺陷因子分(≤17分組與17分組)、多動(dòng)沖動(dòng)因子分(≤13分組與13分組)低分和高分組的比較中差異均無統(tǒng)計(jì)學(xué)意義(均P0.05)。ODD量表9分的患兒DAT1甲基化水平較ODD量表得分≥9分的患兒高(P0.05),且患兒DAT1甲基化水平與ODD量表得分呈負(fù)相關(guān)(r=-2.203,P=0.033)。結(jié)論 DAT1啟動(dòng)子區(qū)甲基化水平可能影響ADHD患者對(duì)立違抗行為的嚴(yán)重程度,且與患者有無抑郁、焦慮和ADHD家族史存在一定關(guān)系。
