p11mRNA與創(chuàng)傷后應(yīng)激障礙的相關(guān)性研究

【摘要】:目的:探討創(chuàng)傷后應(yīng)激障礙(Post Traumatic Stress Disorder,PTSD)患者外周血單個(gè)核細(xì)胞p11m RNA水平的變化及其與PTSD患者臨床癥狀的相關(guān)性。方法:1.研究對(duì)象:收集的病例為2013年10月至2014年12月就診于新疆某大型三級(jí)甲等綜合醫(yī)院的人群,PTSD組為經(jīng)歷創(chuàng)傷事件者,共計(jì)34例。非PTSD組為同期經(jīng)歷創(chuàng)傷事件,但未發(fā)生PTSD的人群,共計(jì)30例。2.研究工具:一般資料調(diào)查問(wèn)卷、創(chuàng)傷后應(yīng)激障礙量表平民版(Post Traumatic Stress Disorder Checklist Civilian Version,PCL-C)、漢密頓焦慮量表(Hamilton Anxiety Scale,HAMA)、漢密頓抑郁量表(Hamilton Depression Scale,HAMD)。3.研究過(guò)程:收集就診于我科門診及住院部中經(jīng)歷過(guò)嚴(yán)重精神創(chuàng)傷事件的人群,進(jìn)行一般情況調(diào)查問(wèn)卷、PCL-C、HAMA、HAMD量表資料的收集。對(duì)于PTSD的診斷由2名精神科醫(yī)生完成。將符合入組標(biāo)準(zhǔn)的人群,在入組當(dāng)日或次日清晨,采集空腹靜脈血,待血樣本收集完成后送至中心實(shí)驗(yàn)室進(jìn)行試驗(yàn)。4.外周血單個(gè)核細(xì)胞p11m RNA水平的測(cè)定:采用實(shí)時(shí)熒光定量聚合酶鏈?zhǔn)椒磻?yīng)(Real-time Quantitative Reverse Transcription Polymerase Chain Reaction,QRT-PCR)檢測(cè)p11m RNA在PTSD組及非PTSD組外周血單個(gè)核細(xì)胞中表達(dá)水平的差異。5.統(tǒng)計(jì)方法:使用“上;菡\(chéng)心理測(cè)驗(yàn)軟件”對(duì)原始資料進(jìn)行轉(zhuǎn)化,使用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行分析,計(jì)數(shù)資料的比較采用χ2檢驗(yàn)。計(jì)量資料進(jìn)行比較時(shí),若為正態(tài)資料,采用t檢驗(yàn);若為非正態(tài)資料,采用非參數(shù)檢驗(yàn)(Nonparametric Tests)Mann-Whitney U檢驗(yàn)法。p11m RNA在各組的表達(dá)水平與臨床特征的相關(guān)性采用Spearson相關(guān)分析。P0.05雙側(cè)檢驗(yàn),為檢驗(yàn)有統(tǒng)計(jì)學(xué)意義。結(jié)果:1.p11m RNA在PTSD組的表達(dá)水平低于非PTSD組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.p11m RNA與PCL-C、HAMA、HAMD量表評(píng)分呈負(fù)相關(guān)(P0.05)。3.p11mRNA與患者年齡、經(jīng)歷創(chuàng)傷事件的年齡及距創(chuàng)傷事件的時(shí)間無(wú)相關(guān)性(P0.05)。結(jié)論:1.PTSD患者外周血單個(gè)核細(xì)胞p11m RNA表達(dá)量低于未發(fā)生PTSD者,外周血單個(gè)核細(xì)胞p11m RNA可能是一個(gè)區(qū)分PTSD患者與非PTSD者的生物學(xué)指標(biāo)。2.經(jīng)歷創(chuàng)傷事件的人群的臨床癥狀越嚴(yán)重,外周血單個(gè)核細(xì)胞p11m RNA表達(dá)量越低。
[Abstract]:Objective: to investigate the changes of p11m RNA in peripheral blood mononuclear cells (PBMC) in patients with post-traumatic stress disorder (Post Traumatic Stress Disorder,PTSD) and its correlation with clinical symptoms of PTSD patients. Method 1: 1. Participants: a total of 34 cases of PTSD were collected from October 2013 to December 2014 in a large Grade 3A General Hospital in Xinjiang. The non PTSD group was a group of 30 patients (30 cases) who had experienced traumatic events but did not have PTSD at the same time. Research tools: general data questionnaire, Post-traumatic stress Disorder (Post Traumatic Stress Disorder Checklist Civilian Version,PCL-C), Hamilton anxiety scale (Hamilton Anxiety Scale,HAMA), Hamilton Depression scale (Hamilton Depression Scale,HAMD). The research process: collect the people who have experienced severe trauma events in outpatient department and inpatient department of our department, and collect the data of PCL-CU Hamah Hamd scale. The diagnosis of PTSD was performed by two psychiatrists. Fasting venous blood was collected on the same day or in the early morning of the next day, and the blood samples were collected and sent to the central laboratory for test. 4. Detection of p11m RNA in peripheral blood mononuclear cells: the expression of p11m RNA in peripheral blood mononuclear cells from PTSD group and non-PTSD group was detected by real-time fluorescence quantitative polymerase chain reaction (Real-time Quantitative Reverse Transcription Polymerase Chain Reaction,QRT-PCR). Statistical methods: the original data were transformed by "Shanghai Huicheng Psychological Test Software" and analyzed by SPSS17.0 statistical software. 蠂 ~ 2 test was used to compare the counting data. When the measurement data were compared, t test was used for normal data, and for non-normal data, the expression level of p11m RNA in each group was tested by (Nonparametric Tests) Mann-Whitney U test. The correlation between the expression level of p11m RNA in each group and clinical characteristics was analyzed by Spearson correlation analysis. P05 bilateral test was used. There is statistical significance for the test. Results the expression of p11m RNA in PTSD group was significantly lower than that in non-PTSD group (P0.05). There was a negative correlation between p11m RNA and PCL-C,HAMA,HAMD score (P0.05) .3.p11mmRNA had no correlation with patient's age, the age of traumatic event and the time to traumatic event (P0.05). Conclusion: 1. The expression of p11m RNA in peripheral blood mononuclear cells in patients with PTSD is lower than that in patients without PTSD. The expression of p11m RNA in peripheral blood mononuclear cells may be a biological marker to distinguish PTSD patients from non-PTSD patients. The more severe the clinical symptoms, the lower the expression of p11m RNA in peripheral blood mononuclear cells.
