LSP-39中文版在精神分裂癥患者中的信效度分析及臨床應(yīng)用研究

【摘要】：目的：考察社會(huì)功能量表(Life Skill Profile-39, LSP-39)中文版在精神分裂癥患者中的信度和效度以及臨床應(yīng)用的研究。 方法：1.收集200例符合DSM-IV-TR精神分裂癥診斷標(biāo)準(zhǔn)的門(mén)診或住院精神分裂癥患者,10例患者用于研究者一致性培訓(xùn),200例進(jìn)行個(gè)體和社會(huì)功能量表(Personal and Social Performance scale, PSP)、陽(yáng)性和陰性癥狀量表(Positive and Negative Syndrome Scale, PANSS)和LSP-39中文版測(cè)評(píng),分析LSP-39量表的內(nèi)部一致性、LSP-39中文版與PSP的一致性(效標(biāo)效度)以及與患者疾病嚴(yán)重度的相關(guān)性(關(guān)聯(lián)效度)。25例患者在首次LSP-39中文版評(píng)估后的21天后,再次對(duì)患者進(jìn)行第2次測(cè)評(píng),評(píng)價(jià)LSP-39中文版的重測(cè)信度。2.采用LSP-39對(duì)296例住院及門(mén)診的精神分裂癥患者社會(huì)功能進(jìn)行臨床研究分析。 結(jié)果：1.LSP-39中文版的內(nèi)部一致性Cron-bachα系數(shù)為0.68-0.84,LSP-39中文版總分的研究者之間一致性Kappa值為0.61,重測(cè)一致性為0.92。LSP-39中文版總分與PSP有較好的相關(guān)性(ICC=0.88),與PANSS總分負(fù)相關(guān)(r=-0.37,P0.01)。 2.從非治療因素看,文化程度高、婚姻穩(wěn)定、社會(huì)支持好的患者社會(huì)功能好。從治療因素看,治療依從性好、近5年住院時(shí)間短、使用副反應(yīng)小的新型抗精神病藥物的患者社會(huì)功能較好。本研究發(fā)現(xiàn)門(mén)診患者社會(huì)功能優(yōu)于住院患者,此外,我們還發(fā)現(xiàn)患者社會(huì)功能與臨床客觀療效判斷變化趨勢(shì)總體一致。 結(jié)論：1.社會(huì)功能量表中文版是評(píng)估住院和門(mén)診精神分裂癥患者社會(huì)功能的一個(gè)簡(jiǎn)潔、易操作、較穩(wěn)定的工具,適用于臨床實(shí)踐或研究中使用。2.要提高精神分裂癥社會(huì)功能應(yīng)當(dāng)更多的考慮綜合因素的作用,尤其強(qiáng)調(diào)早期干預(yù)、社會(huì)支持和藥物、心理相結(jié)合的綜合治療。
[Abstract]:Objective: to investigate the reliability, validity and clinical application of the Chinese version of Social function scale (Life Skill Profile-39, LSP-39) in patients with schizophrenia. Method 1: 1. A total of 200 outpatient or inpatient schizophrenic patients who met the DSM-IV-TR criteria for schizophrenia diagnosis were collected for consistency training in 200 patients with individual and social function scale (Personal and Social Performance scale, PSP), positive and negative symptoms. The Chinese version of (Positive and Negative Syndrome Scale, PANSS) and LSP-39, To analyze the internal consistency of LSP-39 scale and the consistency between LSP-39 Chinese version and PSP, and the correlation between LSP-39 and patient disease severity (correlation validity). Twenty-five patients were evaluated 21 days after the first Chinese version of LSP-39. The patients were tested for the second time to evaluate the retest reliability of the Chinese version of LSP-39. LSP-39 was used to analyze the social function of 296 inpatients and outpatients with schizophrenia. Results 1. The internal consistency Cron-bach 偽 coefficient of the Chinese version of LSP-39 was 0.68-0.84 and the consistent Kappa value of the Chinese version of LSP-39 was 0.61.The retest consistency was that the total score of the Chinese version of 0.92.LSP-39 had a good correlation with PSP (ICC=0.88), and negatively correlated with the total score of PANSS (r-0.37, P0.01). In terms of non-therapeutic factors, patients with high education, stable marriage and good social support have good social function. According to the treatment factors, the patients with good treatment compliance, short hospital stay in the last 5 years and low side effects of new antipsychotics had better social function. In this study, we found that outpatients' social function was better than that of inpatients. In addition, we also found that the social function of outpatients was in accordance with the trend of objective clinical efficacy judgment. Conclusion 1. The Chinese version of the social function scale is a simple, easy to operate and stable tool for assessing the social function of inpatient and outpatient schizophrenic patients. It is suitable for clinical practice or research. In order to improve the social function of schizophrenia, more consideration should be given to the role of comprehensive factors, especially early intervention, social support and combined therapy with drugs and psychology.
