【摘要】：背景:大量研究證據(jù)表明，在重型抑郁癥的治療中，抗抑郁藥物與心理治療有著近乎相當(dāng)?shù)寞熜�。先前研究已用功能磁共振研究過抑郁狀態(tài)及治療產(chǎn)生的腦局部血流變化，但依然沒有證據(jù)表明治療可引起相關(guān)白質(zhì)纖維微結(jié)構(gòu)改變。 目的:利用彌散張量成像技術(shù)，探索成年重型抑郁癥患者接受引導(dǎo)想象心理治療后的特異性白質(zhì)改變。 方法:本研究招募了重型抑郁癥患者21名（16女，5男，平均年齡29.6歲），，于4周心理治療前后各進(jìn)行一次彌散張量成像；與22名健康被試（14女,8男,平均年齡30.2歲）進(jìn)行各向異性分?jǐn)?shù)的比較。 結(jié)果:與健康對照相比，抑郁癥病人的右側(cè)丘腦、小腦蚓部、右側(cè)中央后回及表現(xiàn)出顯著的各向異性分?jǐn)?shù)增高，右側(cè)楔葉降低；四周療程后，左側(cè)額葉白質(zhì)內(nèi)出現(xiàn)了一個各向異性分?jǐn)?shù)增加的區(qū)域。 結(jié)論:接受心理治療的早期階段，在與額葉情緒調(diào)節(jié)相關(guān)的區(qū)域產(chǎn)生了各向異性分?jǐn)?shù)增加，這表明心理治療可以引起額葉的白質(zhì)改變。這可能是通過重塑大腦情緒調(diào)節(jié)網(wǎng)絡(luò)中結(jié)點的聯(lián)接起效的，本研究結(jié)果支持了心理治療起效的『自上而下』機(jī)制。
[Abstract]:Background: there is considerable evidence that antidepressants are almost as effective as psychotherapy in the treatment of severe depression. Previous studies have used functional magnetic resonance to study depression and changes in regional cerebral blood flow resulting from treatment, but there is still no evidence that treatment can lead to changes in the microstructures of white matter fibers. Objective: to explore the specific changes of white matter in adult patients with severe depression after guided psychotherapy with diffuse Zhang Liang imaging. Methods: a total of 21 patients with severe depression (16 females, 5 males, mean age 29.6 years) were enrolled in this study. They were examined by diffusive Zhang Liang imaging before and after 4 weeks of psychotherapy, and 22 healthy subjects (14 females and 8 males) were enrolled in the study. The average age was 30.2 years. Results: compared with healthy controls, the right thalamus, cerebellar vermis, right posterior central gyrus and right posterior gyrus showed a significant increase in anisotropic fraction, and the right cuneate lobe decreased after four weeks of treatment. An area of increased anisotropy appeared in the white matter of the left frontal lobe. Conclusion: in the early stage of psychotherapy, anisotropy scores were increased in the regions associated with emotional regulation in the frontal lobe, indicating that psychotherapy could cause changes in the white matter of the frontal lobe. This may be done by reinventing the connections of nodes in the brain's emotional regulatory network, and the results support the "top-down" mechanism of psychotherapy.
