【摘要】：目的對認(rèn)知障礙初步評價(jià)表(COG-12)在阿爾茨海默病(AD)患者中的信度與效度進(jìn)行評價(jià)。方法選擇2013年1~12月就診于南京醫(yī)科大學(xué)附屬腦科醫(yī)院老年精神科的AD患者148例為AD組,另選擇社區(qū)志愿者365例為對照組,2組均接受臨床癡呆量表、AD8、簡易智能狀態(tài)檢查量表、畫鐘測驗(yàn)評估和COG-12測評。其中100例AD患者知情者在首次評定1個(gè)月內(nèi)重測COG-12。結(jié)果 COG-12的克朗巴赫α系數(shù)=0.883,重測信度=0.883(P0.01)。COG-12各條目評分與總分的相關(guān)系數(shù)為0.41~0.79(P0.05,P0.01);COG-12總分與簡易智能狀態(tài)檢量表、畫鐘測驗(yàn)呈負(fù)相關(guān)(r=-0.67,r=-0.27,P0.01),與臨床癡呆量表、AD8總分呈正相關(guān)(r=0.80,r=0.81,P0.01)。COG-12鑒別認(rèn)知正常與輕度AD患者的ROC曲線下面積=0.902,≥6分為認(rèn)知損害的界限分值敏感性為90.0%,特異性為82.5%;鑒別輕度與中度AD患者的ROC曲線下面積=0.898,敏感性為90.5%,特異性為76.7%。結(jié)論 COG-12具有良好的信度和效度,是快速識(shí)別癡呆及評估嚴(yán)重程度敏感而準(zhǔn)確的篩查評估工具。
[Abstract]:Objective to evaluate the reliability and validity of COG-12 in patients with Alzheimer's disease (AD). Methods from January to December 2013, 148 AD patients in the Department of Geriatrics and Psychiatry, affiliated brain Hospital of Nanjing Medical University were selected as AD group, and 365 community volunteers as control group. Clock drawing test evaluation and COG-12 evaluation. In 100 AD patients, COG-12 was re-tested within 1 month after the first assessment. Results the correlativity coefficient of COG-12 's Cronbach 偽 coefficient was 0.883.The correlation coefficient between each item score and the total score of the retest reliability was 0.883U P0.01U .COG-12 and the correlation coefficient of each item score and the total score of each item was 0.41 / 0.79P0.05P0.05P0.01and the simple intelligence state examination scale. The bell drawing test was negatively correlated with the total score of clinical dementia scale (AD8), and positively correlated with the total score of clinical dementia scale (AD8).
