【摘要】：目的通過檢測首發(fā)精神分裂癥患者(first-episode schizophrenic patients,FEP)服用第二代抗精神病藥物(second generation antipsychotics,SGAs)8周前后的血清脂聯素(adiponectin,APN)及代謝相關指標水平,探討SGAs對首發(fā)精神分裂癥患者APN的影響以及APN在SGAs引發(fā)肥胖中的作用。方法選取86例首發(fā)未服藥的精神分裂癥患者,及88名性別年齡相匹配的正常對照,患者組使用單一SGAs治療8周,測定患者組治療前、治療8周末及對照組的體重、體質指數(body mass index,BMI)、腰臀比(waist-to-hip ratio,WHR)及空腹血糖(fasting plasma glucose,FPG)、甘油三酯(triglyceride,TG)、APN、空腹胰島素(fasting insulin,FINS)等指標。結果患者組在治療前APN水平低于對照組[(9.32±0.76)μg/m L vs.(10.9±0.66)μg/m L],FINS水平高于對照組[(12.68±11.70)μIU/m L vs.(6.47±2.87)μIU/m L],差異有統(tǒng)計學意義(P0.05)。與治療前相比,患者組SGAs治療8周后體重[(59.01±10.56)kg vs.(63.80±9.78)kg]、BMI[(21.74±3.57)kg/m2vs.(23.49±3.44)kg/m2]、WHR[(0.88±0.07)vs.(0.92±0.05)]、TG[(0.94±0.92)mmol/L vs.(1.63±1.08)mmol/L]和FINS水平[(12.68±11.70)μIU/m L vs.(20.27±15.02)μIU/m L]增加,差異均有統(tǒng)計學意義(P0.01),而患者組治療后APN[(9.32±0.76)μg/m L vs.(8.03±0.68)μg/m L]及FPG[(5.04±1.01)mmol/L vs.(4.46±0.57)mmol/L]水平降低,差異均有統(tǒng)計學意義(P0.05)。在男性患者組中,基線APN水平與治療后體重增加值呈正相關(r=0.548,P=0.005),女性患者組中該相關無統(tǒng)計學意義(P0.05)。結論首發(fā)精神分裂癥患者在治療前APN水平降低,SGAs可進一步降低患者APN水平;在男性患者中,基線APN水平對服藥后的體重增加有預測作用。
[Abstract]:Objective to detect the serum adiponectin (APN) and metabolic related indexes of first-time episode schizophrenic patientsof patients with first-episode schizophrenia before and after 8 weeks of administration of the second generation antipsychotic drug, the second generation antipsychoticsof SGAsA, and to determine the level of serum adiponectin (APN) in patients with first-onset schizophrenia before and after 8 weeks of administration of the second-generation antipsychotic drug. To investigate the effect of SGAs on APN in patients with first-episode schizophrenia and the role of APN in SGAs induced obesity. Methods 86 patients with schizophrenia and 88 normal controls matched by sex and age were treated with single SGAs for 8 weeks. The body weight of the patients before treatment, 8 weeks after treatment and the control group were measured.
