~1H-MRS在廣泛性焦慮診斷中的應(yīng)用

【摘要】:目的:探討氫質(zhì)子磁共振波譜(~1H-MRS)對廣泛性焦慮診斷的價值。方法:搜集臨床上確診為廣泛性焦慮的患者24例,健康志愿者24例,行常規(guī)MRI平掃及~1H-MRS掃描。測定雙側(cè)海馬、雙側(cè)前扣帶回、雙側(cè)后扣帶回、雙側(cè)額中回皮質(zhì)、雙側(cè)楔葉皮質(zhì)、雙側(cè)額葉白質(zhì)、雙側(cè)枕葉白質(zhì)區(qū)的N-乙酰天門冬氨酸(NAA)、膽堿化合物(Cho)、肌酸(Cr)、肌醇(Mi)、丙氨酸、葡萄糖峰下面積均值,計算NAA/Cr、Cho/Cr、Mi/Cr、NAA/(Cho+Cr)、Cho/NAA、Lac/Cr值。結(jié)果:病例組雙側(cè)海馬Cho/Cr、Mi/Cr、Cho/NAA較對照組明顯升高,,左側(cè)前扣帶回Cho/Cr及Lac/Cr較對照組升高,雙側(cè)額中回皮質(zhì)Lac/Cr及葡萄糖較對照組升高;右側(cè)后扣帶回MI/Cr較對照組升高;左側(cè)楔葉皮質(zhì)NAA/(Cho+Cr)、Cho/NAA及Lac/Cr較對照組升高,右側(cè)楔葉皮質(zhì)NAA/Cr、Cho/Cr、NAA/(Cho+Cr)、Cho/NAA及葡萄糖較對照組升高;左側(cè)楔葉白質(zhì)Lac/Cr與對照組相比升高,右側(cè)楔葉白質(zhì)的Lac/Cr及葡萄糖較對照組升高;病例組組內(nèi)比較,左側(cè)額葉白質(zhì)NAA/Cr、NAA/(Cho+Cr)較右側(cè)升高,差異有統(tǒng)計學(xué)意義(p㩳0.05)。結(jié)論:廣泛性焦慮患者在常規(guī)腦MRI平掃陰性時,~1H-MRS可以檢測到腦代謝異常,為焦慮癥的定量診斷提供了一定的診斷依據(jù)。
[Abstract]:Objective: to evaluate the value of 1H-MRS in the diagnosis of generalized anxiety. Methods: 24 patients with generalized anxiety and 24 healthy volunteers were examined by conventional MRI scan and 1H-MRS scan. Bilateral hippocampal, bilateral anterior cingulate gyrus, bilateral posterior cingulate gyrus, bilateral middle frontal gyrus cortex, bilateral cuneate cortex, bilateral frontal white matter, bilateral occipital white matter area N- acetyl aspartate NAA, choline compound Chochus, creatine cingulate, inositol Mii, alanine, were measured in bilateral hippocampus, bilateral anterior cingulate gyrus, bilateral cuneiform cortex, bilateral frontal white matter, bilateral occipital white matter area. The average area under the peak of glucose is calculated by calculating the value of NAA / CrCho-Cr / Cr / Cr / NAA / NAA / Cr = Cho / Cr / NAA / AC / Cr value. Results: in the bilateral hippocampus, the Cho/Cr and Lac/Cr of left anterior cingulate gyrus were significantly higher than those in control group, the Lac/Cr and glucose in bilateral middle frontal gyrus were higher than those in control group, and the MI/Cr in right posterior cingulate gyrus was higher than that in control group. The NAA/(Cho / NAA and Lac/Cr in the left cuneiform cortex were higher than those in the control group, and the NAA / NAA and glucose in the right cuneiform cortex were higher than those in the control group, while the Lac/Cr in the left cuneiform white matter was higher than that in the control group, and the Lac/Cr and glucose in the right cuneiform white matter were higher than those in the control group. Compared with the right side, the NAA / CRN NAA / Cho Cr in the left frontal white matter was higher than that in the right side, and the difference was statistically significant (P < 0.05). Conclusion: 1H-MRS can detect abnormal brain metabolism in patients with generalized anxiety when conventional MRI scan is negative, which provides a diagnostic basis for quantitative diagnosis of anxiety disorder.
