【摘要】：背景和目的：目前關(guān)于米氮平聯(lián)合抗精神病藥物針對(duì)精神分裂癥陰性癥狀的治療效果尚無(wú)定論，缺乏有效的證據(jù)。為了更全面系統(tǒng)地了解該藥物合并使用對(duì)精神分裂癥患者的陰性癥狀的療效，本研究通過(guò)循證醫(yī)學(xué)方法為精神分裂癥特別是陰性癥狀的臨床治療提供科學(xué)依據(jù)。方法：通過(guò)系統(tǒng)檢索截止2012年10月份的Pubmed、Embase、中國(guó)知網(wǎng)、萬(wàn)方等相關(guān)數(shù)據(jù)庫(kù)，共檢索到關(guān)于選擇米氮平輔助治療精神分裂癥陰性癥狀患者療效及其安全性的隨機(jī)對(duì)照或臨床對(duì)照試驗(yàn)的相關(guān)文獻(xiàn)共4704篇，按照納入和排除標(biāo)準(zhǔn)進(jìn)行篩選后，最終納入8項(xiàng)隨機(jī)對(duì)照試驗(yàn)。首先，按照J(rèn)adad改良量表的標(biāo)準(zhǔn)，由兩個(gè)人獨(dú)立對(duì)相關(guān)文獻(xiàn)分別進(jìn)行質(zhì)量評(píng)價(jià)；然后，根據(jù)Meta分析的森林圖法對(duì)異質(zhì)性進(jìn)行檢測(cè)，并采取合適的效應(yīng)模型檢驗(yàn)合并效應(yīng)量；最后，利用敏感性分析和漏斗圖法對(duì)存在的偏倚進(jìn)行檢測(cè)。結(jié)果：米氮平聯(lián)合抗精神病藥物治療精神分裂癥陰性癥狀的有效率（OR=3.75[2.73,5.16]）和痊愈率（OR=2.21[1.23,3.99]）明顯要高于單獨(dú)使用抗精神病藥物治療組�？傮w上看米氮平聯(lián)合抗精神病藥物組的不良發(fā)生率與單獨(dú)使用抗精神病藥物組差別沒(méi)有統(tǒng)計(jì)學(xué)意義（P0.05），除實(shí)驗(yàn)組惡心與失眠這兩項(xiàng)副反應(yīng)的發(fā)生率明顯低于對(duì)照組（P0.05）；嗜睡這一副反應(yīng)發(fā)生率低于單用抗精神病藥物組（P0.05），，只有失眠的發(fā)生率明顯低于單用抗精神病藥物組（P0.01）。結(jié)論：米氮平聯(lián)合抗精神病藥物對(duì)于精神分裂癥陰性癥狀的治療具有明顯的效果，同時(shí)，聯(lián)合用藥并未顯著增加常見(jiàn)的抗精神藥物所帶來(lái)的副反應(yīng)發(fā)生率。
[Abstract]:Background and objective: at present, there is no conclusion on the efficacy of metozapine combined with antipsychotics in the treatment of negative symptoms of schizophrenia. In order to understand the efficacy of the combined use of the drug on the negative symptoms of schizophrenia, this study provides a scientific basis for the clinical treatment of schizophrenia, especially negative symptoms, through the method of evidence-based medicine. Methods: the relevant databases such as Pubmedum Embase, China knowledge Network, Wanfang and so on, were searched systematically by means of systematic search for the month of October, 2012, etc. A total of 4704 articles of randomized controlled or clinical controlled trials on the efficacy and safety of metozapine in adjuvant treatment of schizophrenia with negative symptoms were retrieved and screened according to the inclusion and exclusion criteria. Finally, 8 randomized controlled trials were included. First of all, according to the standard of Jadad improvement scale, two people independently evaluate the quality of relevant literature. Then, according to the forest map method of Meta analysis, the heterogeneity is tested, and the appropriate effect model is adopted to test the combined effect quantity. Sensitivity analysis and funnel graph were used to detect the bias. Results: the effective rate of metozapine combined with antipsychotics in the treatment of schizophrenia negative symptoms was 3.75 [2.735.16] and the cure rate was 2.21 [1.233.99], which was significantly higher than that in the group treated with antipsychotic drugs alone. On the whole, there was no significant difference in the incidence of adverse effects between the metozapine combined with antipsychotics group and the antipsychotic drug group alone (P 0.05). The incidence of adverse reactions except nausea and insomnia in the experimental group was significantly lower than that in the control group. The incidence of somnolence was lower than that of single antipsychotics group, and the incidence of insomnia was significantly lower than that of single antipsychotics group. Conclusion: metozapine combined with antipsychotics has obvious effect on the treatment of negative symptoms of schizophrenia, and the incidence of side effects caused by common antipsychotic drugs has not been significantly increased by the combination of mizapine and antipsychotic drugs.
