【摘要】:目的通過對(duì)92例精神分裂癥患者(SP組)和74名無嚴(yán)重軀體及免疫性疾病的健康對(duì)照者(HC組)染色體體型畸變(CA)率、微核(MN)發(fā)生率、IL-1α、IL-1β、IL-2、IL-6、IL-17、部分激素及血清糖脂代謝水平的觀測(cè),探討內(nèi)分泌-免疫網(wǎng)絡(luò)失衡與精神分裂癥的相關(guān)性,為精神分裂癥的早期診斷、治療及預(yù)后提供實(shí)驗(yàn)依據(jù)。方法1.基本信息及各項(xiàng)體征指數(shù)問卷調(diào)查及病理資料查詢。2.全自動(dòng)免疫分析儀化學(xué)發(fā)光微粒子免疫分析(CMIA)法檢測(cè)HPT軸、HPA軸激素水平;酶聯(lián)免疫(ELISA)法檢測(cè)IL-1α、IL-1β、IL-2、IL-6、IL-17水平;外周血淋巴細(xì)胞CA實(shí)驗(yàn)、MN實(shí)驗(yàn)檢測(cè)遺傳物質(zhì)損傷;全自動(dòng)生化分析儀(酶法)檢測(cè)糖、脂代謝水平。結(jié)果1.SP組各項(xiàng)體征指數(shù)及高血壓患病率與HC組無明顯差異;HDL明顯低于、GLU、TG、TC、LDL顯著高于HC組;LDL、HDL與病程分別呈正、負(fù)相關(guān)。2.SP組HPT軸激素中,T4水平明顯高于HC組,且T4與病程、PANSS正相關(guān)(r=0.072,0.087;p均0.05);HPA軸激素中僅CORT水平明顯高于HC組,且與PANSS(r=0.495,p0.05)正相關(guān)。3.兩組IL-2、IL-6水平無明顯差異(p0.05);SP組IL-1α、IL-1β、IL-17明顯高于HC組,且IL-1α、IL-1β、IL-17無性別、城鄉(xiāng)、精神疾病家族史差異,且與年齡不相關(guān)(p0.05),IL-1α、IL-1β與PANSS顯著正相關(guān)(r=0.266,0.286;p均0.01),IL-17與總病程及PANSS分值正相關(guān)(r=0.459,0.228;p均0.05)。4.SP組染色體體型畸變(CA)率、微核(MN)發(fā)生率、CA與MN異常人數(shù)與HC組均無明顯差異(p0.05)。結(jié)論1.HDL降低,LDL、T4、CORT增高,說明內(nèi)分泌代謝紊亂與精神分裂癥相關(guān)。2.精神分裂癥病人未發(fā)現(xiàn)遺傳物質(zhì)的異常結(jié)構(gòu)損傷。3.精神分裂癥患者IL-1α、IL-1β、IL-17細(xì)胞因子增高;其中IL-17在精神分裂癥的發(fā)生發(fā)展過程中起到重要的作用,在精神分裂癥的早期診斷及療效評(píng)價(jià)中具有一定參考價(jià)值。4.神經(jīng)-內(nèi)分泌-免疫網(wǎng)絡(luò)失衡與精神分裂癥發(fā)生密切相關(guān),調(diào)控內(nèi)分泌免疫紊亂對(duì)該病的治療具有重要的意義。
[Abstract]:Objective to observe the rate of chromosome type aberration (CAA) and the incidence of MNs in 92 patients with schizophrenia (SP group) and 74 healthy controls without severe somatic and immunological diseases (HC group), and to observe the incidence of IL-1 偽, IL-1 尾, IL-2, IL-6, IL-17, some hormones and the level of serum glucose and lipid metabolism. To explore the relationship between endocrine immune network imbalance and schizophrenia, and to provide experimental basis for early diagnosis, treatment and prognosis of schizophrenia. Method 1. Basic information and physical signs index questionnaire and pathological information inquiry. 2. HPT axis hormone levels were detected by chemiluminescence microparticle immunoassay (CMIA), IL-1 偽 IL-1 尾 / IL-2IL-6 / IL-17 by enzyme linked immunosorbent assay (Elisa), and genetic material damage by peripheral blood lymphocyte CA assay (MN). The level of glucose and lipid metabolism was detected by automatic biochemical analyzer (enzymatic method). Results there was no significant difference between 1.SP group and HC group in the index of signs and the prevalence of hypertension. The level of T4 in HPT axis in 1.SP group was significantly higher than that in HC group, and the level of T4 in HPT axis of SP group was significantly higher than that in HC group. There was a positive correlation between T4 and PANSS, and there was a positive correlation between T4 and PANSS (P < 0.05). The level of only CORT in HPA-axis was significantly higher than that in HC group, and it was positively correlated with PANSS (0.495p0.05). There was no significant difference in the level of IL 2 and IL 6 between the two groups. The levels of IL 17 in IL-1 偽 and IL 1 尾 and IL 17 in SP group were significantly higher than those in HC group, and there were no gender differences in IL 2 尾 IL 17 between urban and rural areas and the family history of mental illness. In addition, there was a significant positive correlation between IL-1 尾 and PANSS with age independent (P < 0.05). The positive correlation between IL-17 and the total course of disease and the PANSS score were 0.4590.228p and 0.4590.228p respectively. There was no significant difference in the incidence of micronucleus MNs and the number of abnormal CA and MN in HC group (P 0.05). Conclusion 1.HDL decreased the increase of T 4 Cort, indicating that endocrine metabolic disorder is associated with schizophrenia. No abnormal structural damage of genetic material was found in schizophrenic patients. The levels of IL-1 偽, IL-1 尾 and IL-17 cytokines were increased in schizophrenic patients, in which IL-17 played an important role in the occurrence and development of schizophrenia, and had a certain reference value in the early diagnosis and evaluation of curative effect of schizophrenia. The imbalance of neuroendocrine and immune network is closely related to the occurrence of schizophrenia. The regulation of endocrine immune disorder is of great significance to the treatment of the disease.
