酒精成癮者靜息態(tài)默認(rèn)網(wǎng)絡(luò)fMRI研究

【摘要】:目的:應(yīng)用靜息態(tài)功能磁共振成像的獨(dú)立成分分析及種子相關(guān)分析方法,研究酒精成癮者默認(rèn)網(wǎng)絡(luò)改變及其內(nèi)異常腦區(qū)功能連接的改變,以及功能連接改變與臨床變量的關(guān)系,探討酒精成癮的可能神經(jīng)機(jī)制。 方法:采用3.0T磁共振對(duì)30例酒精成癮者與30例性別、年齡、受教育程度及尼古丁依賴程度匹配健康志愿者進(jìn)行靜息態(tài)功能磁共振掃描,采取獨(dú)立成分分析方法提取默認(rèn)網(wǎng)絡(luò),對(duì)兩組默認(rèn)網(wǎng)絡(luò)進(jìn)行統(tǒng)計(jì)分析得到默認(rèn)網(wǎng)絡(luò)對(duì)比差異區(qū)域;選取差異區(qū)域中的一重要區(qū)域作為種子點(diǎn)進(jìn)行功能連接分析,且將功能連接強(qiáng)度與臨床量表進(jìn)行Pearson相關(guān)分析。 結(jié)果:相對(duì)于對(duì)照組,酒精成癮組默認(rèn)網(wǎng)絡(luò)在右楔前葉、左小腦腳Ⅱ區(qū)功能連接減低,而在左后扣帶回、右顳中回、左顳上回及右楔前葉區(qū)域功能連接增強(qiáng);左小腦腳Ⅱ區(qū)與雙側(cè)楔前葉、右內(nèi)側(cè)額葉、左額上回之間的功能連接減弱,而與左小腦半球Ⅵ區(qū)、雙側(cè)腦島、雙側(cè)頂下小葉之間的功能連接增強(qiáng),,且左小腦腳Ⅱ區(qū)與雙側(cè)腦島功能連接強(qiáng)度與飲酒時(shí)間呈正相關(guān)。 結(jié)論:酒精成癮者默認(rèn)網(wǎng)絡(luò)功能異常,且默認(rèn)網(wǎng)絡(luò)內(nèi)左小腦腳Ⅱ區(qū)參與的神經(jīng)環(huán)路異常,結(jié)合相關(guān)分析結(jié)果,提示靜息態(tài)功能磁共振成像對(duì)于酒精成癮復(fù)雜神經(jīng)機(jī)制的揭示及臨床評(píng)估具有一定的價(jià)值。
[Abstract]:Objective: to study the changes of default network and functional connections in abnormal brain regions of alcohol addicts and the relationship between the changes of functional connections and clinical variables by means of independent component analysis and seed correlation analysis of resting functional magnetic resonance imaging (fMRI).To explore the possible neural mechanism of alcohol addiction.Methods: 30 patients with alcohol addiction and 30 healthy volunteers with sex, age, education and nicotine dependence were examined with 3.0T magnetic resonance imaging (MRI), and the default network was extracted by independent component analysis (ICA).Through the statistical analysis of two groups of default networks, the contrast area of the default network is obtained, and an important area in the difference area is selected as the seed point for functional connection analysis, and the Pearson correlation analysis between the strength of functional connection and the clinical scale is carried out.Results: compared with the control group, the default network in the alcohol addiction group was decreased in the anterior lobe of right wedge, and the functional connections in the left posterior cingulate gyrus, the right middle temporal gyrus, the left superior temporal gyrus and the right anterior cuneate lobe were enhanced.The functional connections between the left cerebellum 鈪
