【摘要】：目的 通過(guò)對(duì)不同程度的腦卒中后抑郁(post stroke depression,PSD)病人腦部前額葉功能激活變化特點(diǎn)的觀察,進(jìn)一步探討能否利用近紅外光腦成像技術(shù)(functional near-infrared spectroscopy,f NIRS)協(xié)助早期識(shí)別及輔助診斷腦卒中后抑郁狀態(tài)奠定一定的研究基礎(chǔ)。方法 應(yīng)用近紅外光腦成像系統(tǒng)對(duì)腦卒中后抑郁狀態(tài)病人(輕度抑郁組27例,中度抑郁組19例,重度抑郁組18例)和腦卒中后未出現(xiàn)抑郁或情緒不穩(wěn)的病人(對(duì)照組29例)在執(zhí)行語(yǔ)義言語(yǔ)流暢任務(wù)(verbal fluency task,VFT)時(shí),測(cè)定前額葉腦組織氧合血紅蛋白(oxygenate hemoglobin,Oxy-Hb)峰值濃度和達(dá)峰時(shí)間,并進(jìn)行比較分析,對(duì)相關(guān)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果 輕度抑郁組病人Oxy-Hb峰值濃度為(0.29±0.17)mmol·L~(-1)·mm,達(dá)峰時(shí)間為(74.27±24.12)s,中度抑郁組病人Oxy-Hb峰值濃度為(0.17±0.11)mmol·L~(-1)·mm,達(dá)峰時(shí)間為(95.14±70.35)s,重度抑郁組病人Oxy-Hb峰值濃度為(0.16±0.30)mmol·L~(-1)·mm,達(dá)峰時(shí)間為(125.95±25.68)s;輕度抑郁組病人兩方面與對(duì)照組差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);中度抑郁組病人僅在Oxy-Hb峰值濃度方面與對(duì)照組差異有統(tǒng)計(jì)學(xué)意義(P0.05);只有重度抑郁組病人在以上兩方面與對(duì)照組差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 Oxy-Hb峰值濃度是一個(gè)較為敏感的指標(biāo),可用于中度、重度PSD病人的觀察,達(dá)峰時(shí)間只對(duì)重度病人適用,對(duì)于輕度病人上述兩個(gè)指標(biāo)均不適用;PSD后抑郁程度不同的病人,具有的抑郁情緒癥狀不同,而呈現(xiàn)出前額葉功能激活下降狀態(tài)的生物學(xué)特征也有差異,重度抑郁狀態(tài)的病人前額葉有氧代謝功能失調(diào)情況更嚴(yán)重,表現(xiàn)更為典型,這將為進(jìn)一步研究將近紅外光腦成像檢查作為PSD早期識(shí)別及輔助診斷的一種檢查方法奠定一定的研究基礎(chǔ)。
[Abstract]:Objective to observe the changes of prefrontal lobe function in patients with post post stroke depression (PSD) after stroke.To further explore whether the near infrared photoencephalography (NIR) technique can be used to assist in the early identification and diagnosis of post-stroke depression.Methods Near-infrared light brain imaging system was used to treat 27 patients with depression after stroke (mild depression group, 19 patients with moderate depression group, 19 patients with moderate depression group).In 18 patients with severe depression and 29 patients without depression or mood instability after stroke (control group, 29 patients), the peak concentration and peak time of oxygenated hemoglobulin Oxy-Hb in prefrontal lobe brain tissue were measured when performing semantic speech fluency task.And carries on the comparative analysis, carries on the statistical analysis to the correlation result.緇撴灉 杞誨害鎶戦儊緇勭梾浜篛xy-Hb宄板，
