【摘要】：目的：利用基于低頻振幅算法（ALFF）的靜息功能磁共振(fMRI)技術(shù)探討無(wú)癡呆型血管性認(rèn)知障礙(VCIND)與正常老年人靜息狀態(tài)下基線腦活動(dòng)變化。 方法：22名VCIND患者作為研究對(duì)象，23名正常老年人（NC）作為對(duì)照組，采用低頻振幅（the amplitude of low-frequency fluctuations，ALFF）分析方法，比較兩組平均腦ALFF圖的差異；提取兩組被試雙側(cè)后扣帶回/楔前葉（PCC/Pcu）的ALFF值，檢驗(yàn)VCIND組與正常對(duì)照組之間是否有統(tǒng)計(jì)學(xué)差異,并將VCIND組PCC/PCu的ALFF值與MoCA評(píng)分進(jìn)行相關(guān)分析。 結(jié)果：1.VCIND患者在靜息狀態(tài)下較對(duì)照組在以下腦區(qū)ALFF值增高,包括左額下回/左顳上回（Z=-4.0682)、右額下回/右顳上回（Z=-3.9720)、左側(cè)島葉（Z=-3.5773)；VCIND患者組在以下腦區(qū)出現(xiàn)ALFF值減低:舌回/小腦后葉（Z=3.8921）、前扣帶皮層（Z=3.8081）、右楔葉/右舌回（Z=4.8272）、后扣帶回/楔前葉（Z=3.9741）（P0.05)；2.兩組被試PCC/Pcu的ALFF值分別為：VCIND組0.9861±0.0192，，正常對(duì)照組0.9984±0.0207，正常對(duì)照組較VCIND組ALFF值更高（兩樣本t檢驗(yàn)t=2.063，P=0.0450.05）；3.VCIND組PCC/PCu的ALFF值與MoCA評(píng)分相關(guān)分析，相關(guān)系數(shù)R=0.455，方差分析F=5.22，P=0.033,可認(rèn)為VCIND組患者PCC/PCu的ALFF值與MoCA評(píng)分呈正相關(guān)。 結(jié)論：VCIND患者在靜息狀態(tài)下ALFF有顯著改變，主要表現(xiàn)為語(yǔ)言中樞等認(rèn)知區(qū)域的局部ALFF增強(qiáng)及DMN等功能連接點(diǎn)的ALFF減低。
[Abstract]:Aim: to investigate the baseline brain activity changes of VCIND without dementia type vascular cognitive impairment (VCIND) and the rest state of normal elderly by using resting functional magnetic resonance (Mr) technique based on low frequency amplitude algorithm (ALFF). Methods Twenty two patients with VCIND were selected as control group. The low frequency amplitude of the amplitude of low-frequency structures (ALF) was used to compare the difference of average brain ALFF between the two groups. The ALFF values of the posterior cingulate gyrus / anterior cuneate lobe of the two groups were extracted to test whether there was a statistical difference between the VCIND group and the normal control group. The correlation between the ALFF value of the PCC/PCu and the MoCA score in the VCIND group was analyzed. Results 1. The ALFF values of VCIND patients in the rest state were higher than those in the control group in the following brain regions. In the left inferior frontal gyrus / left superior temporal gyrus Z-4.0682A, right inferior frontal gyrus / right superior temporal gyrus Z-3.9720, the left insular gyrus Z-3.5773V VCIND patients had decreased ALFF values in the following areas: lingual gyrus / posterior lobe Z3.8921, anterior cingulate cortex Z3.8081, right cuneate / right lingual gyrus Z4.8272, posterior cingulate gyrus / cuneate gyrus / posterior cingulate gyrus / cuneate gyrus. The ALFF value of PCC/Pcu in the two groups was 0.9861 鹵0.01922 in the control group and 0.9984 鹵0.0207 in the normal control group. The ALFF value in the normal control group was higher than that in the VCIND group. The correlation coefficient was 0.455, and the variance analysis of variance was 5.22%. It was suggested that the ALFF value of PCC/PCu was positively correlated with the MoCA score in the VCIND group. Conclusion there were significant changes in ALFF in patients with VCIND at rest, mainly manifested by the enhancement of local ALFF in cognitive regions such as language center and the decrease of ALFF in functional junctions such as DMN.
