【摘要】：目的:探討烏靈膠囊聯(lián)合米氮平治療更年期抑郁癥的療效以及對(duì)患者神經(jīng)內(nèi)分泌的影響。方法:采用隨機(jī)雙盲對(duì)照法將我院195例更年期抑郁患者隨機(jī)分為對(duì)照組和治療組,對(duì)照組給予米氮平治療,治療組給予米氮平+烏靈膠囊治療。治療8周,觀察并比較兩組患者的漢密爾頓抑郁量表(HAMD)評(píng)分和改良Kupperman指數(shù)評(píng)分(KMI)、世界衛(wèi)生組織生存質(zhì)量測(cè)定量表簡(jiǎn)表(WHOQOL-BREF)評(píng)分、不良反應(yīng)癥狀量表(TESS)。治療前后測(cè)定并比較兩組患者血清單胺類神經(jīng)遞質(zhì)5-羥色胺(5-TH)、去甲腎上腺素(NE)、雌二醇(E2)、促卵泡激素(FSH)、促黃體生成素(LH)水平。結(jié)果:對(duì)照組和治療組的有效率分別為80.61%和92.78%(P0.01)。治療后2、4、6、8周與治療前HADM評(píng)分比較2組均有明顯降低,從治療第2周末起,治療組的HADM總分較對(duì)照組明顯下降。治療后兩組間的KMI評(píng)分和WHOQOL-BREF評(píng)分比較均有顯著性差異。治療后兩組患者的5-羥色胺(5-TH)和去甲腎上腺素(NE)較治療前明顯升高;治療后兩組患者的雌二醇(E2)、促卵泡激素(FSH)、促黃體生成素(LH)濃度較治療前均有顯著性差異。對(duì)照組的不良反應(yīng)總數(shù)18例(18.37%),TESS值為(5.89±0.41)分;治療組的不良反應(yīng)總數(shù)8例(8.25%),TESS評(píng)分(3.14±0.28)分。治療組的不良反應(yīng)數(shù)及TESS明顯低于對(duì)照組。結(jié)論:烏靈膠囊可提高米氮平治療更年期抑郁癥臨床療效,且不良反應(yīng)少,依從性好,并能有效地調(diào)整患者的E2、FSH、LH水平,提高患者生存質(zhì)量。
[Abstract]:Objective: to investigate the curative effect of Wuling capsule combined with miazapine on climacteric depression and its effect on neuroendocrine. Methods: 195 patients with climacteric depression in our hospital were randomly divided into control group and treatment group by randomized double-blind control. The control group was treated with mirtazapine, while the treatment group was treated with miazapine Wuling capsule for 8 weeks. To observe and compare the scores of Hamilton Depression scale (Hamd), modified Kupperman Index (Kupperman) and World Health Organization quality of Life scale (WHOQOL-BREFF) between the two groups. Before and after treatment, the serum levels of monoamine neurotransmitters 5-hydroxytryptamine 5-THN, norepinephrine, estradiol E _ 2, follicle stimulating hormone FSHN, luteinizing hormone (LH) were measured and compared between the two groups before and after treatment. The effective rates of the treatment group were 80.61% and 92.78, respectively. Compared with the HADM score before treatment, the HADM scores of the two groups were significantly lower than that of the control group. From the end of the second week of treatment, The total score of HADM in the treatment group was significantly lower than that in the control group. There were significant differences in KMI score and WHOQOL-BREF score between the two groups after treatment. After treatment, the concentrations of estradiol estradiol, follicle stimulating hormone (FSH) and luteinizing hormone (LH) in the two groups were significantly different from those before treatment. The total number of adverse reactions in the control group was 18.3710% (5.89 鹵0.41). The number of adverse reactions and TESS in the treatment group were significantly lower than those in the control group. Conclusion: Wuling capsule can improve the clinical efficacy of metozapine in the treatment of menopausal depression, and the adverse reactions are less and the compliance is good. And can effectively adjust the level of E _ 2 FSH LH and improve the quality of life of patients.
