【摘要】：目的對精神分裂癥患者在不同治療階段的工作狀況建立預(yù)測和分類模型,以探討其工作狀況的預(yù)測因素。方法收集隨機(jī)分到綜合治療組和常規(guī)藥物治療組的170例精神分裂癥患者在基線及3、6、12個(gè)月的工作狀況和人口學(xué)、社會和臨床指標(biāo),用分類回歸樹的方法建立預(yù)測模型。結(jié)果基線時(shí)首次發(fā)病年齡預(yù)測患者是否工作的分割點(diǎn)是25歲,模型正確估計(jì)了患者總數(shù)的67.6%;隨訪3個(gè)月時(shí),自知力水平總分大于13分、接受綜合治療和受教育年限大于9年可以產(chǎn)生82.6%的節(jié)點(diǎn)(工作的患者),模型正確估計(jì)了患者總數(shù)的73.4%;隨訪6個(gè)月和12個(gè)月時(shí),個(gè)人和社會功能大于70或72分。病程或受教育年限是預(yù)測工作狀況的主要指標(biāo),模型總體預(yù)測準(zhǔn)確率分別為74.8%和73.7%。結(jié)論急性期的患者其工作狀況主要和首次發(fā)病年齡、精神癥狀嚴(yán)重程度有關(guān);恢復(fù)期患者工作狀況主要和自知力的水平、是否接受綜合治療有關(guān);鞏固期患者工作狀況主要和社會功能的恢復(fù)情況、病程以及受教育程度有關(guān)。
[Abstract]:Objective to establish a prediction and classification model for the work status of schizophrenic patients in different stages of treatment. Methods 170 schizophrenic patients who were randomly assigned to the comprehensive treatment group and the routine drug treatment group were collected at baseline and in 3 months and 12 months of work status and demography. Social and clinical indicators, using the method of classification and regression tree to establish a prediction model. Results the first onset of age at baseline to predict whether the patient work division point is 25 years old, the model correctly estimated the total number of patients 67.6%; At 3 months follow-up, the total score of insight level was more than 13 points, and the number of nodes (working patients) could be 82.6% after comprehensive treatment and education for more than 9 years. The model correctly estimated 73.4% of the total number of patients. At 6 and 12 months follow-up, the individual and social function were more than 70 or 72 points. Course of disease or length of education were the main predictors of work status. The overall predictive accuracy of the model was 74.8% and 73.7 respectively. Conclusion the work status of the patients in the acute stage is mainly related to the age of the first onset and the severity of mental symptoms. The working condition of convalescent patients is mainly related to the level of insight and whether or not they accept comprehensive treatment. The work status of patients in consolidation period is mainly related to the recovery of social function, the course of disease and the level of education.
