不同部位起搏對(duì)正常和心力衰竭犬心室跨壁復(fù)極梯度的影響(英文)

跨壁復(fù)極梯度的改變可能導(dǎo)致跨壁復(fù)極離散度的增加和室速的增多,跨壁復(fù)極離散可能在左室心外膜起搏相關(guān)猝死中發(fā)生重要作用。本研究對(duì)心力衰竭(心衰)犬進(jìn)行心室不同部位起搏,觀察左心室跨壁復(fù)極梯度的變化。選用8條健康雜種犬,隨機(jī)分成健康對(duì)照組和心衰組(n=4),心衰組4條健康犬經(jīng)快速右心室心內(nèi)膜心尖部(right ventricular apical endocardium,RVEndo)起搏4~5周建立慢性充血性心衰模型。健康對(duì)照組和心衰組分別在右心房(right atrium, RA)起搏、RVEndo起搏、左心室心外膜(left ventricular lateral epicardium, LVEpi)起搏及雙心室(biventricular, Biv)同步起搏的條件下,應(yīng)用自制的跨室壁單相動(dòng)作電位(monophasic action potential, MAP)記錄電極,于左心室同步記錄和測(cè)量三層心。▋(nèi)層、中層、外層)的MAP時(shí)程(MAP duration, MAPD)。結(jié)果顯示,健康對(duì)照組竇性心律時(shí)左心室三層心肌M...
