體內(nèi)移植羊膜對(duì)家兔肌腱損傷模型肌腱愈合的影響

【摘要】:研究的目的在于觀察羊膜體內(nèi)移植對(duì)肌腱損傷修復(fù)的影響。60只跟腱損傷家兔動(dòng)物模型采用同體對(duì)照動(dòng)物實(shí)驗(yàn)方法,大體觀察傷口愈合、肌腱粘連及肌腱肥大情況,并分別于術(shù)后第2、4、6周等3個(gè)時(shí)間點(diǎn)取跟腱,HE染色觀察損傷區(qū)域炎細(xì)胞浸潤(rùn)和成纖維細(xì)胞數(shù)量,免疫組化染色觀察膠原纖維排列塑形狀況,拉伸試驗(yàn)測(cè)定肌腱彈性模量。另取4只家兔不做肌腱切斷術(shù)作為正常對(duì)照,在第0周測(cè)定肌腱彈性模量。全部實(shí)驗(yàn)動(dòng)物皮膚切口在術(shù)后1周內(nèi)基本愈合,未發(fā)生傷口感染情況。羊膜實(shí)驗(yàn)組跟腱粘連動(dòng)物數(shù)量和肌腱斷裂區(qū)域肥大程度低于陰性對(duì)照組(P0.05)。術(shù)后第2周,兩組動(dòng)物炎癥反應(yīng)均達(dá)到最高值,但羊膜實(shí)驗(yàn)組炎細(xì)胞(包括中性粒細(xì)胞和單核細(xì)胞)浸潤(rùn)數(shù)量低于陰性對(duì)照組(P0.05)。兩組實(shí)驗(yàn)動(dòng)物樣本中成纖維細(xì)胞數(shù)量和膠原纖維積累數(shù)量無(wú)明顯差異(P0.05),但羊膜實(shí)驗(yàn)組成纖維細(xì)胞和膠原纖維排列更具有序性,尤其是在術(shù)后第6周,明顯好于陰性對(duì)照組。術(shù)后第2周和第4周,羊膜實(shí)驗(yàn)組肌腱彈性模量均優(yōu)于陰性對(duì)照組(P0.05)。羊膜具有促進(jìn)肌腱早期愈合和抗炎、抗粘連作用。
[Abstract]:The purpose of this study was to observe the effect of amniotic membrane transplantation on the repair of tendon injury. Sixty rabbit models of Achilles tendon injury were used to observe the wound healing, tendon adhesion and tendon hypertrophy.The HE staining of Achilles tendon was used to observe the infiltration of inflammatory cells and the number of fibroblasts in the injured area, the arrangement and shape of collagen fibers were observed by immunohistochemical staining, and the elastic modulus of tendons was measured by tensile test.The elastic modulus of tendon was measured at 0 th week in 4 rabbits without tendon amputation as normal control.All experimental animal skin incisions healed basically within 1 week after operation, and no wound infection occurred.The number of achilles tendon adhesions and the degree of hypertrophy of tendon rupture in the amniotic membrane group were lower than those in the negative control group (P 0.05).At the second week after operation, the inflammatory reaction reached the highest level in both groups, but the infiltration of inflammatory cells (including neutrophils and monocytes) in the amniotic membrane experimental group was lower than that in the negative control group (P 0.05).There was no significant difference in the number of fibroblasts and the accumulation of collagen fibers between the two groups, but the arrangement of fibroblasts and collagen fibers in the amniotic membrane experimental group was more orderly than that in the negative control group, especially at the 6th week after operation.At the 2nd and 4th week after operation, the elastic modulus of tendon in the amniotic membrane group was better than that in the negative control group (P 0.05).Amniotic membrane has the function of promoting early healing, anti-inflammation and anti-adhesion of tendon.
