【摘要】：目的介紹一種離斷性腎盂成形術(shù)的腹腔鏡訓練模型。方法采用豬腎、雞食道嗉囊制作成腎盂輸尿管連接部狹窄模型,雞軀干用于固定該模型。在自制腹腔鏡模擬訓練箱內(nèi),對8名完成腹腔鏡基本技能培訓但無實際腹腔鏡手術(shù)經(jīng)驗的年輕醫(yī)師進行10輪的模擬訓練,記錄每次手術(shù)的完成時間,并根據(jù)每次手術(shù)術(shù)中縫合撕裂針數(shù),術(shù)后檢查吻合口針距、邊距的情況,累計成手術(shù)的失誤評分,對吻合口質(zhì)量進行初步評估。結(jié)果 8名年輕醫(yī)師均成功完成了培訓,手術(shù)時間從最初72.70±8.26 min降至最后的54.50±7.69 min;吻合口失誤評分從最初5.60±1.35分降至最后的2.50±1.58分,差異均有統(tǒng)計學意義(P0.05)。結(jié)論此模型制作簡易、術(shù)野逼真、訓練有效,能較好地模擬腹腔鏡離斷性腎盂成形術(shù)的各個手術(shù)步驟,適合于臨床醫(yī)師進行該術(shù)式的初級培訓。
[Abstract]:Objective to introduce a laparoscopic training model of amputated pyeloplasty. Methods the model of ureteropelvic junction stenosis was made from pig kidney and chicken oesophagus crop, and the torso of chicken was used to fix the model. Eight young physicians who had completed basic skills training in laparoscopic surgery but had no practical experience in laparoscopic surgery were trained for 10 rounds of simulated training. The time of completion of each operation was recorded, and the number of laceration needles sutured during each operation was recorded. After operation, the needle distance and margin of anastomosis were examined, and the error score of operation was accumulated. The quality of anastomotic site was evaluated preliminarily. Results all the 8 young doctors successfully completed the training. The operative time decreased from 72.70 鹵8.26 min to 54.50 鹵7.69 min, and the anastomotic error score decreased from 5.60 鹵1.35 to 2.50 鹵1.58. The difference was statistically significant (P 0.05). Conclusion this model is simple, realistic and effective. It can simulate each procedure of laparoscopic disconnected pyeloplasty and is suitable for the primary training of clinicians.
