【摘要】：經(jīng)自然孔道內(nèi)鏡手術(shù)(Natural Orifice Translumenal Surgery,NOTES)是指通過人體與外界自然相通的腔道如胃、結(jié)腸、陰道及膀胱等,以內(nèi)鏡置入體腔完成手術(shù)操作的微創(chuàng)治療方法。NOTES技術(shù)包括混合NOTES(hybrid NOTES,HNOTES)及完全NOTES(pure NOTES,PNOTES)兩類。與傳統(tǒng)開放手術(shù)相比,NOTES的技術(shù)特點和優(yōu)勢在于無體表傷口,機體損傷小,麻醉需求量少,傷口并發(fā)癥少等。目前可能的手術(shù)穿刺位點包括經(jīng)胃、經(jīng)口、經(jīng)陰道、經(jīng)直腸及經(jīng)膀胱途徑。經(jīng)陰道途徑切口操作相對簡單,可在直視下完成,切口閉合容易,并發(fā)癥少,是目前NOTES技術(shù)的主要手術(shù)穿刺位點。目前泌尿外科經(jīng)陰道NOTES(transvaginal pure NOTES,TVPNOTES)技術(shù)大多采用經(jīng)腹入路。目的:探索標(biāo)準(zhǔn)腹腔鏡器械行腹膜后入路經(jīng)陰道純NOTES(TVPNOTES)腎切除的安全性和可行性。方法:1.雌豬4只,平均體重31kg,氣管全麻后切開陰道后外側(cè)壁,鈍性擴張陰道內(nèi)切口并分離陰道周圍組織,置入自制擴張氣囊初步擴張腹膜后空間,經(jīng)陰道內(nèi)切口安裝TriPort建立操作通道,置入腹腔鏡行腹膜后探查術(shù),觀察術(shù)中及術(shù)后2周并發(fā)癥情況。2.雌豬6只,平均體重30kg,氣管全麻后行陰道后外側(cè)壁切開,鈍性擴張陰道內(nèi)切口,并分離陰道周圍組織,置入自制擴張氣囊初步擴張腹膜后空間,經(jīng)陰道內(nèi)切口安裝改良型TriPort建立操作通道,置入腹腔鏡后于直視下鈍性、銳性結(jié)合分離腹膜后組織至腎臟平面,參照腹腔鏡腎切除術(shù)切除腎臟,取出腎臟后間斷縫合陰道內(nèi)切口。術(shù)后2周觀察生命體征及并發(fā)癥情況。1.4例實驗均完成了陰道切開,其中3例TriPort安裝成功,并完成了腹膜后探查。置入TriPort成功的3例實驗均出現(xiàn)了明顯的切口漏氣情況,無法維持有效的氣腹壓力,術(shù)中術(shù)后未觀察到并發(fā)癥。結(jié)果:2.2例實驗早期出現(xiàn)腹膜損傷而中止手術(shù),1例在輔助Trocar的幫助下成功切除并取出腎臟,3例成功完成標(biāo)準(zhǔn)腹腔鏡器械腹膜后經(jīng)陰道NOTES腎切除術(shù),術(shù)中術(shù)后未見并發(fā)癥。結(jié)論:1.標(biāo)準(zhǔn)腹腔鏡器械行豬動物模型腹膜后經(jīng)陰道純NOTES腎切除術(shù)安全可行。2.改良的TriPort可作為豬模型腹膜后經(jīng)陰道純NOTES技術(shù)操作平臺。
[Abstract]:Natural Orifice Translumenal Surgerynot (NOTES) refers to the natural communication between the human body and the outside world, such as stomach, colon, vagina and bladder. There are two kinds of minimally invasive treatment methods. Notes: mixed NOTES(hybrid NOTES (HNOTES) and complete NOTES(pure NOTES (PNOTES). Compared with traditional open surgery, the technical features and advantages of NOTES are that it has no surface wound, less injury and less anesthetic demand. The possible sites of surgical puncture include transgastric, transoral, transvaginal, transrectal and transvesical approaches. The transvaginal incision is relatively simple and can be completed under direct vision. The incision is easy to close and has fewer complications. At present, transvaginal NOTES(transvaginal pure NOTES (TVPNOTES) is the main operative puncture site of NOTES technique. Objective: to explore the safety and safety of retroperitoneal retroperitoneal approach for transpaginal nephrectomy with standard laparoscopic instruments. Method: 1. Four female pigs, The average body weight was 31 kg. After general trachea anesthesia, the posterior lateral wall of vagina was cut open, the inner incision of vagina was dilated passively and the perivaginal tissue was separated. The self-made dilated airbag was placed into the primary dilated retroperitoneal space, and TriPort was installed through the incision of the vagina to establish the operation channel. Retroperitoneal exploration was performed by laparoscopy. Complications during and 2 weeks after operation were observed in 6 female pigs with an average body weight of 30 kg. After general trachea anesthesia, posterior lateral vaginal wall incision, blunt expansion of the vaginal incision, and separation of perivaginal tissue were performed. The retroperitoneal space was initially dilated by self-made dilated airbags, and modified TriPort was installed through the intravaginal incision to establish the operation channel. After the placement of laparoscopy, blunt peritoneal tissue was obtuse under direct vision, and the retroperitoneal tissue was separated from the renal plane by sharp combination. With reference to laparoscopic nephrectomy, the kidney was removed and the intravaginal incision was sutured intermittently. The vital signs and complications of 1. 4 cases were observed 2 weeks after operation. Vaginal incision was completed in 3 cases, among which 3 cases were successfully installed with TriPort. The results of retroperitoneal exploration were performed in 3 cases successfully implanted with TriPort. All of them had obvious incision leakage and could not maintain effective pneumoperitoneum pressure. Results there was no postoperative complication in 2. 2 cases with peritoneal injury in the early stage of the experiment, 1 case was successfully resected with the help of Trocar and 3 cases were successfully performed retroperitoneal NOTES nephrectomy with standard laparoscopic instruments, 1 case was successfully resected with the help of Trocar, 3 cases were successfully performed retroperitoneal NOTES nephrectomy with standard laparoscopic instruments.
