【摘要】：【目的】研究重組人松弛素(recombinant human relaxin,rh-RLX)對(duì)舒張性心力衰竭(Distolic Heart Failure,DHF)模型兔心臟功能的影響。【方法】1.選取36只成年新西蘭家兔,按數(shù)字表法隨機(jī)分為手術(shù)組(n=30只)和假手術(shù)組(n=6),采用腹主動(dòng)脈縮窄術(shù)(縮窄50%-60%)建立DHF動(dòng)物模型,假手術(shù)組僅開(kāi)腹游離出腹主動(dòng)脈但不縮窄。術(shù)后第8周行動(dòng)物心臟超聲心動(dòng)圖檢查,根據(jù)2007年歐洲心臟病學(xué)會(huì)及超聲協(xié)會(huì)推出的DHF診斷標(biāo)準(zhǔn):左心室射血分?jǐn)?shù)(LVEF)㧐50%且二尖瓣舒張?jiān)缙谘鞣逯?舒張晚期血流峰值(E/A㩳1)為DHF建模成功標(biāo)準(zhǔn)。2.將建模成功的手術(shù)組家兔隨機(jī)分為DHF組(n=6)、洛丁新組(n=8,給藥劑量:3mg/(kg·d),)、小劑量組(n=8,給藥劑量:rh-RLX 30ug/(kg·d))、大劑量組(n=8,給藥劑量:rh-RLX 98ug/(kg·d)),DHF組與假手術(shù)組正常飼養(yǎng)作空白對(duì)照組,各組連續(xù)給藥1周。3.所有分組家兔于術(shù)前、術(shù)后第8周、術(shù)后第9周三個(gè)時(shí)間點(diǎn)行心臟超聲心動(dòng)圖檢查,通過(guò)成組資料T檢驗(yàn)與配對(duì)樣本T檢驗(yàn)比較各組家兔心臟功能變化情況。4.術(shù)后第9周,將上述5組家兔分別在行心臟超聲心動(dòng)圖檢查后,空氣栓塞法處死各組家兔,分別留取左心室心肌在我院病理科制作家兔心肌病理切片并采用Masson染色法觀察各組家兔心肌間質(zhì)纖維增生變化情況。【結(jié)果】1.心臟超聲心動(dòng)圖結(jié)果:(1).術(shù)后第8周,與假手術(shù)組相比,手術(shù)組家兔心臟收縮功能指標(biāo)左心室射血分?jǐn)?shù)(LVEF)(76.97±3.61vs75.90±1.67,p㧐0.05)正常,差異無(wú)統(tǒng)計(jì)學(xué)意義,而各舒張功能指標(biāo)具有統(tǒng)計(jì)學(xué)差異,表現(xiàn)為左心室后壁厚度(LVPW)(2.48±0.25mm vs 4.86±0.17mm,p㩳0.01)、室間隔厚度(IVS)(2.72±0.31mm vs3.20±0.10mm,p㩳0.01)明顯增厚、E/A比值(1.55±0.31vs0.84±0.07,p㩳0.01)明顯下降、左心室舒張末壓(LVEDP)(4.09±0.42mm Hg vs 11.7±1.26mm Hg,p㩳0.01)顯著升高;(2)術(shù)后第9周,與DHF組比較,大劑量組E/A顯著增加(1.36±0.10 vs 0.83±0.06,p㩳0.01),LVPW(3.71±0.21mm vs 4.87±0.08mm,p㩳0.01)、IVS(3.10±0.08mm vs3.23±0.08mm,p㩳0.05)、LVEDP(8.74±1.16 mm Hg vs 12.07±1.31mm Hg,p㩳0.01)均顯著降低。而與DHF組比較,洛丁新組和小劑量組各心功能指標(biāo)均無(wú)顯著差異(p㧐0.05);(3)術(shù)后第9周,與洛丁新組比較,大劑量組E/A(1.36±0.10 vs 0.87±0.05,p㩳0.05)顯著增加、LVPW(3.71±0.21mm vs 4.75±0.18mm,p㩳0.01)及LVEDP(8.74±1.16mm Hg vs 12.07±1.31 mm Hg,p㩳0.01)均顯著下降,余心功能指標(biāo)均無(wú)顯著差異性(p㧐0.05),而與洛丁新組比較,小劑量組各心功能指標(biāo)均無(wú)顯著差異性(p㧐0.05)。2.心肌Masson染色結(jié)果顯示:洛丁新組、rh-RLX小劑量組、rh-RLX大劑量組心肌間質(zhì)纖維增生狀況較DHF組均有不同程度減輕,其中改善最顯著的是rh-RLX大劑量組,主要表現(xiàn)在該組增生的心肌間質(zhì)纖維顯著減少僅殘存少許纖維條,心肌細(xì)胞內(nèi)浸潤(rùn)的慢性炎癥細(xì)胞完全消失,心肌細(xì)胞體積及胞質(zhì)形態(tài)均完全恢復(fù)正常�！窘Y(jié)論】本研究結(jié)果證實(shí)采用大劑量rh-RLX治療DHF模型兔,可明顯減輕DHF兔心肌纖維化程度從而改善心臟舒張功能,其機(jī)制可能與rh-RLX具有抗心肌纖維化功能密切相關(guān)。
[Abstract]:[Objective] to study the effects of recombinant human relaxin (recombinant human, relaxin, rh-RLX) on diastolic heart failure (Distolic Heart Failure, DHF) on cardiac function in rabbit model. [Methods] 1. select 36 adult New Zealand rabbits were randomly divided into operation group (n= = 30) and sham operation group (n=6), coarctation of the abdominal aortic (narrow 50%-60%) to establish an animal model of DHF, the sham operation group only open free abdominal aortic coarctation. But not after eighth weeks for animal echocardiography, according to the diagnostic criteria of DHF in 2007 the European Society of Cardiology and ultrasound Association launched: left ventricular ejection fraction (LVEF)? 50% and mitral early diastolic peak / late diastolic peak flow (E/A? 1) DHF modeling success criteria.2. will modeling successful surgical group rabbits were randomly divided into DHF group (n=6), Lotensin group (n=8 dosage: 3mg/ (kg, d), small) dose group ( N=8 dose: rh-RLX 30ug/ (kg - D), high dose group (n=8), dose: rh-RLX 98ug/ (kg, d)), DHF group and sham operation group were normal control group, each group for 1 weeks in all groups of rabbits in.3. before surgery, eighth weeks postoperatively after Wednesday, after ninth time points for echocardiography by grouped data T test and paired samples T test were compared ninth weeks of rabbit cardiac function changes after.4., the 5 groups of rabbits were in echocardiography, air embolism death rabbits were collected, left ventricular in pathology department rabbit myocardial pathology and the Masson were observed in rabbits of myocardial interstitial fibrosis changes stain method.
