【摘要】：目的觀察重癥監(jiān)護(hù)室(ICU)中急性呼吸窘迫綜合征患者非甲狀腺疾病綜合征的發(fā)病情況,探討其與患者病情及預(yù)后的相關(guān)性。方法選取43例入住ICU的急性呼吸窘迫綜合征患者,檢測入住ICU時的甲狀腺素水平,分別統(tǒng)計(jì)并比較非甲狀腺疾病綜合征患者和正常甲狀腺功能者的機(jī)械通氣時間、ICU住院時間、28 d死亡率,分析相關(guān)影響因素。結(jié)果 43例急性呼吸窘迫綜合征患者中28例(65.11%)伴非甲狀腺疾病綜合征,與正常甲狀腺功能者比較,非甲狀腺疾病綜合征患者機(jī)械通氣時間、ICU住院時間明顯延長(P0.05),28 d死亡率顯著升高(P0.01),非甲狀腺疾病綜合征與28 d死亡率明顯相關(guān)。結(jié)論急性呼吸窘迫綜合征伴非甲狀腺疾病綜合征的診斷,對預(yù)測患者病情嚴(yán)重程度和預(yù)后有重要價值。
[Abstract]:Objective to observe the incidence of non-thyroid disease syndrome (NDS) in patients with acute respiratory distress syndrome (ARDS) in intensive care unit (ICU) and to explore its correlation with the patient's condition and prognosis. Methods Forty-three patients with acute respiratory distress syndrome (ICU) were enrolled in this study. Thyroxine levels were measured at the time of admission to ICU. The duration of mechanical ventilation and the length of stay of ICU in patients with non-thyroid disease syndrome and normal thyroid function were calculated and compared. 28-day mortality rate was analyzed and the related influencing factors were analyzed. Results of the 43 patients with acute respiratory distress syndrome, 28 (65.11%) were complicated with non-thyroid disease syndrome. Compared with those with normal thyroid function, the duration of mechanical ventilation in the patients with non-thyroid disease syndrome was significantly higher than that in the patients with normal thyroid function. The hospitalization time of ICU was significantly prolonged (P0.05), the mortality was significantly increased on the 28th day (P0.01), and the non-thyroid disease syndrome was significantly correlated with the 28-day mortality. Conclusion the diagnosis of acute respiratory distress syndrome with non-thyroid disease syndrome is of great value in predicting the severity and prognosis of patients with acute respiratory distress syndrome.
