游離DNA完整性檢測對良惡性胸腹腔積液鑒別診斷的價(jià)值研究

【摘要】:目的探討游離DNA(cfDNA)的完整性(ALU247/ALU115)檢測對良惡性胸腹腔積液鑒別診斷價(jià)值。方法采用實(shí)時熒光定量PCR方法,測定86份良惡性胸腹腔積液標(biāo)本中ALU115和ALU247片段濃度并計(jì)算完整性;同時對標(biāo)本中LDH、AFP、CEA、SF、SCC等傳統(tǒng)指標(biāo)進(jìn)行檢測。繪制受試者工作曲線(ROC曲線),根據(jù)曲線下面積評估各標(biāo)記物診斷意義;運(yùn)用統(tǒng)計(jì)學(xué)方法,與傳統(tǒng)指標(biāo)進(jìn)行對比,評價(jià)cf DNA完整性在良惡性胸腹腔積液鑒別診斷中的應(yīng)用價(jià)值。結(jié)果惡性組DNA完整性指數(shù)(ALU247/ALU115)顯著高于良性組,差異具有統(tǒng)計(jì)學(xué)意義(P0.01),ROC曲線下面積為0.70。cfDNA濃度和完整性聯(lián)合檢測的靈敏度、特異度及準(zhǔn)確度均顯著高于其他各項(xiàng)指標(biāo)單獨(dú)或聯(lián)合檢測。該聯(lián)合檢測方式與cfDNA聯(lián)合傳統(tǒng)指標(biāo)在鑒別診斷良惡性胸腹腔積液方面比較差異無統(tǒng)計(jì)學(xué)意義。結(jié)論 cfDNA完整性檢測是良惡性胸腹腔積液鑒別診斷可能的良好靶標(biāo)。ALU115、ALU247片段濃度和完整性(ALU247/ALU115)三項(xiàng)聯(lián)合檢測診斷價(jià)值更高。
[Abstract]:Objective to investigate the value of free DNA (cfDNA) integrity (ALU247/ALU115) in differential diagnosis of benign and malignant pleural and peritoneal effusion. Methods the concentrations of ALU115 and ALU247 fragments in 86 samples of benign and malignant pleural and peritoneal effusion were measured by real-time fluorescence quantitative PCR and the computational integrity was calculated. The traditional indexes such as LDH,AFP,CEA,SF,SCC in the samples were also detected. The subjects' operating curve (ROC curve) was drawn and the diagnostic significance of each marker was evaluated according to the area under the curve. To evaluate the value of cf DNA integrality in differential diagnosis of benign and malignant pleural and peritoneal effusion. Results the DNA integrity index (ALU247/ALU115) of malignant group was significantly higher than that of benign group, and the difference was statistically significant (P0.01), ROC curve area was the sensitivity of 0.70.cfDNA concentration and integrity combined detection. Specificity and accuracy were significantly higher than other indicators alone or combined detection. There was no significant difference in differential diagnosis of benign and malignant pleural and peritoneal effusion between this method and cfDNA combined with traditional indexes. Conclusion cfDNA integrity detection is a good target for differential diagnosis of benign and malignant pleural and peritoneal effusion, and the combined detection of ALU115,ALU247 fragment concentration and integrity (ALU247/ALU115) is more valuable.
