【摘要】：目的探討常規(guī)治療以及常規(guī)治療加用纖維支氣管鏡下支氣管灌洗治療老年吸入性肺炎(AP)的臨床療效。方法選取2012年12月至2013年12月在該院接受治療的AP老年患者52例,其中男32例,女20例,按照是否同意行纖維支氣管鏡下支氣管灌洗分為兩組,對照組(25例)給予常規(guī)治療(包括普通吸痰管吸痰),實驗組(27例)給予常規(guī)治療加用纖維支氣管鏡下支氣管灌洗,持續(xù)觀察至出院,觀察兩組患者治療后第3天的白細胞總數(shù)(WBC)及氧合指數(shù)(PaO2/FiO2)、治療第7天X線胸片中炎癥病灶的變化。結(jié)果治療第3天,兩組PaO2/FiO2上升,WBC下降,兩組治療方法均有效,且實驗組療效明顯優(yōu)于對照組(P0.05)。治療第7天復(fù)查胸片,實驗組有21例肺部陰影面積縮小≥50%,對照組有11例實驗組的治療方法明顯優(yōu)于對照組(P0.05)。結(jié)論兩組治療后PaO2/FiO2、WBC以及肺部陰影明顯改善。證明在老年AP患者的治療中纖維支氣管鏡支氣管灌洗的療效明顯優(yōu)于普通吸痰管吸痰。
[Abstract]:Objective to investigate the clinical effect of routine treatment and bronchial lavage under fiberoptic bronchoscopy in the treatment of senile aspiration pneumonia (AP). Methods from December 2012 to December 2013, 52 elderly AP patients, including 32 males and 20 females, were divided into two groups according to their consent to bronchial lavage under fiberoptic bronchoscopy. The control group (25 cases) received routine treatment (including common sputum suction tube), the experimental group (27 cases) received routine treatment plus bronchofibroscope broncholavage, and continued observation until discharge. The total (WBC) and oxygenation index (PaO2/FiO2) of leukocytes were observed on the 3rd day after treatment, and the changes of inflammatory lesions in chest radiographs on the 7th day after treatment were observed. Results on the 3rd day of treatment, PaO2/FiO2 increased and WBC decreased in both groups. Both groups were effective, and the curative effect of the experimental group was significantly better than that of the control group (P0.05).
