【摘要】：目的調(diào)查廣東地區(qū)急性上呼吸道感染患者并發(fā)病毒性心肌炎的現(xiàn)狀,分析其易患影響因素,為臨床防控尤其是急診一線的防控提供依據(jù)。方法選取廣東藥科大學(xué)附屬第一醫(yī)院、北京大學(xué)深圳醫(yī)院及廣州市第一人民醫(yī)院診斷為病毒性心肌炎的患者120例列入病例組,并隨機(jī)選擇同期上述3家醫(yī)院的單純急性上呼吸道感染患者120例列入對照組。調(diào)查并比較兩組患者的一般情況,日常生活、工作、行為方式和患病情形,并采用logistic回歸分析篩選出急性上呼吸道感染并發(fā)病毒性心肌炎的獨(dú)立影響因素。結(jié)果年齡(x~2=9.866,P=0.007)、月工資收入(x~2=9.572,P=0.023)、睡眠時間(x~2=10.486,P=0.005)、工作時間(x~2=15.495,P=0.000)、生活壓力(x~2=9.760,P=0.008)、就業(yè)壓力(x~2=8.921,P=0.003)、醫(yī)療保障類型(x~2=8.877,P=0.031)、身體素質(zhì)(x~2=9.347,P=0.009)、患病病程(x~2=40.652,P=0.000)、發(fā)熱情況(x~2=54.024,P=0.000)、患病后運(yùn)動負(fù)荷(x~2=16.229,P=0.000)、就診態(tài)度(x~2=11.886,P=0.003)與急性上呼吸道感染并發(fā)病毒性心肌炎有一定的相關(guān)性,其中發(fā)熱(OR=2.323,95% CI 1.234~4.373,P=0.009)是獨(dú)立影響因素。結(jié)論青壯年人群、失業(yè)、低收入、睡眠不足、工作超時、生活壓力、缺乏必要的醫(yī)療保障、身體素質(zhì)差、病程遷延、拖延就診、患病后運(yùn)動負(fù)荷大可增加本地區(qū)急性上呼吸道感染并發(fā)病毒性心肌炎的易患性,而中、高度發(fā)熱作為獨(dú)立影響因素則可進(jìn)一步加大發(fā)病的風(fēng)險,臨床上應(yīng)給予高度重視并積極防治。
[Abstract]:Objective to investigate the current situation of viral myocarditis in patients with acute upper respiratory tract infection in Guangdong province and analyze its risk factors so as to provide evidence for clinical prevention and control, especially for emergency front line prevention and control. Methods 120 patients with viral myocarditis were selected from the first affiliated Hospital of Guangdong Pharmaceutical University, Shenzhen Hospital of Peking University and the first people's Hospital of Guangzhou. 120 patients with simple upper respiratory tract infection were randomly selected as control group. To investigate and compare the general situation, daily life, work, behavior and illness of the two groups, and to screen out the independent influencing factors of acute upper respiratory tract infection complicated with viral myocarditis by logistic regression analysis. Results Age (x / 2 / 9.866 / P), monthly wage income (x ~ (2) / 9.572 / P ~ (0.023), sleep time (x ~ (2) = 10.486 / P ~ (0.005), working time (x ~ (2) ~ (15.495) / P ~ (0.000), stress of living (x ~ (29) 760 / P ~ (0.008), respectively. Employment stress (x / o 2 / 8.921 / P), type of medical security (x / o = 8.877 / P = 0.031), physical quality (x / o = 9.347 / P = 0.009), course of illness (x = 2n = 40.652 / P = 0.000), fever (x = 254.024 / P = 0.000), physical quality (x / o = 9.347 / P = 0.009), course of illness (x = 40.652n = 0.000), or fever. The exercise load after the illness (x2 + 16.229g / kg), attitude to visit the doctor (xF211.886) and acute upper respiratory tract infection complicated with viral myocarditis were associated with fever (OR=2.323,95% CI 1.234 鹵4.373, P < 0.05), and there was a significant correlation between the load of exercise and the attitude to visit the hospital (XM211.886, P0. 003), among which fever (OR=2.323,95% CI 1.234 鹵4.373). P0. 009) is an independent factor. Conclusion Young and middle-aged people, unemployed, low income, lack of sleep, overtime, stress of life, lack of necessary medical security, poor physical quality, course of disease delay, delay to see a doctor. Exercise load after illness can increase the susceptibility of acute upper respiratory tract infection complicated with viral myocarditis in this area, while high fever as an independent influencing factor can further increase the risk of the disease. We should attach great importance to the prevention and treatment in clinic.
