通過(guò)脈沖震蕩系統(tǒng)評(píng)價(jià)COPD的呼吸阻抗

【摘要】:目的脈沖震蕩可評(píng)估肺阻力和電抗,我們的目的是去研究COPD中哪一個(gè)IOS參數(shù)與氣道阻力、小氣道功能和殘氣量相關(guān)。方法 180例COPD患者入組,95例隨訪1年。IOS參數(shù)(R5、R20、X5、Fres),肺體描記法(FEF75、TLC、RV、Rtot)和肺量計(jì)法(FEV1)被測(cè)量。Pearson和Spearman相關(guān)法評(píng)價(jià)IOS和其他測(cè)量法的關(guān)系。結(jié)果 R5、X5、Fres與FEV1、FEF75、RV、Rtot顯著相關(guān),而R20除與FEF75、Rtot低度相關(guān)(相關(guān)系數(shù)小于0.25)外,與其他參數(shù)均無(wú)相關(guān)性。1年后,FEV1、X5較前增大,RV減小,差異均有顯著性。其他參數(shù)1年前后差異無(wú)顯著性。FEV1、RV1年前后的變化值與R5、X5、Fres的變化值相關(guān),而與R20的變化值不相關(guān)。結(jié)論 IOS電抗值X5比阻力值與COPD其他肺功能參數(shù)關(guān)系更密切。IOS電抗參數(shù)對(duì)于氣流阻塞引起的肺順應(yīng)性的改變具有指示作用。
[Abstract]:Objective to evaluate pulmonary resistance and reactance by pulse oscillation. Our aim is to study which IOS parameters in COPD are related to airway resistance, small airway function and residual volume. Methods patients with COPD were enrolled and 95 patients were followed up for one year. The parameters of IOS (R5N R20X5Fres), pulmonary tracheography (FEF75,TLC,RV,Rtot) and pulmonary meter method (FEV1) were measured. Pearson and Spearman correlation methods were used to evaluate the relationship between IOS and other measurement methods. Results there was a significant correlation between R5 and FEV1,FEF75,RV,Rtot, but R20 had no correlation with other parameters except FEF75,Rtot (correlation coefficient < 0. 25). After 1 year, FEV1,X5 increased and RV decreased. The difference was significant. There was no significant difference between the other parameters before and after one year, but the change value of FEV1,RV1 year was correlated with the change value of R5X5Fres, but not with the change value of R20. Conclusion the IOS reactance value X5 is more closely related to other pulmonary function parameters of COPD than the resistance value. IOS reactance parameter can indicate the change of lung compliance caused by airflow obstruction.
