用積分模型診斷結(jié)核性胸膜炎的價(jià)值研究

【摘要】:目的探討應(yīng)用積分模型診斷結(jié)核性胸膜炎的價(jià)值,以提高胸腔積液的診斷率。方法分析65例結(jié)核性胸腔積液和87例惡性胸腔積液患者的臨床資料,選取年齡、體溫、胸腔積液中的腺苷脫氨酶(ADA)、血清C-反應(yīng)蛋白(CRP)4個(gè)指標(biāo)建立積分模型,以年齡37歲、體溫37.8℃、ADA45U/L或30U/L、CRP26mg/L為截點(diǎn),根據(jù)logistic回歸方程相關(guān)性設(shè)置每個(gè)指標(biāo)合適的分值,根據(jù)每例患者的指標(biāo)分值,建立積分模型,利用積分模型作為判斷結(jié)核性胸腔積液的依據(jù)。結(jié)果以積分模型分值≥7分作為預(yù)測(cè)結(jié)核性胸腔積液的截點(diǎn),其敏感性為95.4%、特異性為96.6%、準(zhǔn)確性為96.0%。結(jié)論積分模型可以簡(jiǎn)單方便的進(jìn)行結(jié)核性胸腔積液的診斷。
