穩(wěn)定期COPD患者外周血中Th17和Treg的表達(dá)及平衡關(guān)系變化

【摘要】:目的觀察輔助性T細(xì)胞17(Th17)和調(diào)節(jié)性T淋巴細(xì)胞(Treg)在穩(wěn)定期慢性阻塞性肺疾病(COPD)患者外周血中的表達(dá)及平衡關(guān)系變化,探討其在COPD發(fā)病機(jī)制的作用。方法選取22例肺功能正常不吸煙者(A組)、22例肺功能正常吸煙者(B組)以及36例穩(wěn)定期COPD患者(C組)作為研究對(duì)象。運(yùn)用流式細(xì)胞術(shù)檢測(cè)外周血提取外周單個(gè)核細(xì)胞(PBMC)中的Th17和Treg的表達(dá)及平衡關(guān)系的變化。結(jié)果 C組外周血Th17的表達(dá)率明顯高于B組及A組,差異有統(tǒng)計(jì)學(xué)意義(P0.01),B組Th17細(xì)胞外周血中的表達(dá)高于A組(P0.01);C組外周血中Treg細(xì)胞的表達(dá)率高于A組(P0.01)和B組(P0.01),B組高于A組,差異有統(tǒng)計(jì)學(xué)意義(P0.01);Th17/Treg比值C組明顯高于A組和B組,A組和C組的差異有統(tǒng)計(jì)學(xué)意義(P0.01),但A組與B組及B組與C組的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3組數(shù)據(jù)匯總分析外周血中Th17細(xì)胞和Treg細(xì)胞的表達(dá)與吸煙量呈正相關(guān)(P0.01),Th17/Treg的比值與吸煙量無(wú)明顯相關(guān)性(P0.05);3組數(shù)據(jù)分析Th17細(xì)胞、Treg細(xì)胞以及Th17/Treg比值與第1秒用力呼氣容積(FEV1)/預(yù)計(jì)值(%)和FEV1/用力肺活量(FVC)(%)呈負(fù)相關(guān)。結(jié)論 C組外周血中Th17細(xì)胞和Treg細(xì)胞表達(dá)水平明顯升高,可能參與COPD的發(fā)病機(jī)制;C組的Th17/Treg比值升高,平衡失調(diào),可能是COPD發(fā)病的免疫機(jī)制;Th17和Treg細(xì)胞表達(dá)水平與吸煙量呈正相關(guān),與FEV1/預(yù)計(jì)值(%)及FEV1/FVC(%)呈負(fù)相關(guān)。
[Abstract]:Objective to observe the expression and balance of helper T cell 17 (Th17) and regulatory T lymphocyte (Treg) in the peripheral blood of patients with chronic obstructive pulmonary disease (COPD) at stable stage, and to explore its role in the pathogenesis of COPD. Methods 22 patients with normal pulmonary function (group A), 22 patients with normal pulmonary function (group B) and 36 patients with stable COPD (group C) were studied. Flow cytometry was used to detect the expression of Th17 and Treg in peripheral blood extracted peripheral mononuclear cells (PBMC). Results the expression rate of Th17 in peripheral blood of group C was significantly higher than that of group B and group A, and the difference was statistically significant (P0.01), B group was higher than group A (P0.01); C group) in Th17 cell expression rate was higher than A group (P0.01); C group) and B group (P0.01), B group was higher than A group). The ratio of Th17/Treg in group C was significantly higher than that in group A and group B (P0.01), but there was no significant difference between group A and group B and group C (P0.05). The expression of Th17 cells and Treg cells was positively correlated with smoking volume (P0.01), while the ratio of Th17/Treg was not significantly correlated with smoking volume (P0.05), and the ratio of Th17 cells, Treg cells and Th17/Treg was negatively correlated with forced expiratory volume (FEV1) / predictive value (%) and forced vital capacity (FVC) (%) of FEV1/. Conclusion the expression of Th17 cells and Treg cells in peripheral blood of group C is significantly increased, which may be involved in the pathogenesis of COPD, and the ratio of Th17/Treg in group C is increased and the balance is not balance. The expression levels of Th17 and Treg cells were positively correlated with smoking and negatively correlated with the predicted value of FEV1/ (%) and FEV1/FVC (%).
