脈沖振蕩技術評價塵肺患者肺功能的應用價值

【摘要】:目的探討脈沖振蕩肺功能(IOS)評價塵肺患者肺功能的臨床價值。方法對119例塵肺患者和79例健康者依次進行IOS及PFT檢測,分析兩組人群兩種肺功能各項指標之間的差異、相關性。結果對于早期(壹期)塵肺組,PFT中FEV1、FVC、FEV1/FVC、MVV較健康對照組無明顯差異,MEF25、MEF50、MEF75則明顯降低,差異有統(tǒng)計學意義(P0.05);而IOS參數(shù)中Z5、X5、AX明顯升高,差異有統(tǒng)計學意義(P0.05)。在PFT正常的塵肺患者中,壹期塵肺組的IOS參數(shù)較健康對照組無明顯差異,而貳叁期塵肺組的Z5、X5、Fres則明顯升高,差異有統(tǒng)計學意義(P0.05);壹期塵肺組和貳叁期塵肺組IOS異常率均較PFT高,差異有統(tǒng)計學意義(P0.05);IOS參數(shù)Z5、R5、R20、R5-R20、Fres、AX與PFT指標FVC、FEVl、FEVl/FVC、MVV、MEF25、MEF50、MEF75呈負相關,X5則呈正相關,兩種肺功能指標相關系數(shù)(絕對值)按由高到低依次為:Fres與FEV1/FVC(r=-0.566)、Fres與FEVl(r=-0.557)、Z5與FEVl/FVC(r=-0.544)、AX與FEVl/FVC(r=-0.541)、R5與FEVl/FVC(r=-0.541),在貳叁期塵肺組中相關性更高。結論 IOS可以發(fā)現(xiàn)早期塵肺患者的氣道阻力異常,其操作簡便、對受檢者配合度要求低,可能是塵肺患者肺功能評價的有益補充。
[Abstract]:Objective to evaluate the clinical value of pulsed oscillatory pulmonary function (IOS) in evaluating pulmonary function in patients with pneumoconiosis. Methods IOS and PFT were detected in 119 patients with pneumoconiosis and 79 healthy subjects respectively. The difference and correlation between the two indexes of pulmonary function were analyzed. Results there was no significant difference in FEV1,FVC,FEV1/FVC,MVV between the early stage and the control group (P 0.05), but there was a significant difference between the two groups (P0.05), while the IOS parameter of Z5F5AX was significantly higher than that of the control group (P0.05).
