【摘要】：目的探討痰液、支氣管肺泡灌洗液(BALF)及血液半乳甘露聚糖(GM)對(duì)侵襲性肺曲霉病(IPA)的診斷價(jià)值差異。方法選擇100例慢阻肺(COPD)患者,其中43例為IPA組,57例為非IPA組。半定量測(cè)定痰液、BALF及血清GM抗原,比較GM抗原對(duì)IPA診斷敏感度及特異度的差異。結(jié)果 COPD患者IPA繼發(fā)感染組痰液、BALF及血清GM的A值均顯著高于非IPA患者;以≥0.5為陽(yáng)性界值時(shí),痰液、BALF及血清GM敏感度、特異度、陽(yáng)性預(yù)測(cè)值和陰性預(yù)測(cè)值分別為74.4%、82.4%、76.2%、81.0%;83.7%、87.7%、83.7%、87.7%;48.8%、94.7%、87.5%、71.1%。以1.0為陽(yáng)性界值時(shí),BALF和血清GM特異度、陽(yáng)性預(yù)測(cè)值均達(dá)100.0%,而痰液GM特異度達(dá)91.2%,但痰液和BALF GM的敏感度和陰性預(yù)測(cè)值明顯高于血清。結(jié)論痰液GM以0.5為陽(yáng)性界值,對(duì)COPD患者IPA繼發(fā)感染的診斷價(jià)值與BLAF相似,略優(yōu)于血液,且無(wú)侵入性操作帶來(lái)的痛苦,有較高的應(yīng)用價(jià)值。
[Abstract]:Objective to investigate the diagnostic value of sputum, bronchoalveolar lavage fluid (BALF) and galactomannan (GM) (GM) in the diagnosis of invasive pulmonary aspergillosis (IPA). Methods A total of 100 patients with chronic obstructive pulmonary disease (COPD) were selected, of whom 43 were IPA group and 57 were non-IPA group.
