【摘要】：目的探討布地奈德霧化吸入治療老年慢性阻塞性肺疾病(COPD)急性加重期患者的療效及對患者肺功能、降鈣素原(PCT)和C反應(yīng)蛋白(CRP)水平的影響。方法 230例老年COPD急性加重期患者隨機數(shù)字表法分為布地奈德組(n=117)和對照組(n=113),兩組均給予止咳、平喘、抗感染、低流量吸氧等常規(guī)治療,在常規(guī)治療的基礎(chǔ)上對照組給予甲潑尼龍琥珀酸鈉治療,在對照組治療的基礎(chǔ)上給予布地奈德組布地奈德霧化吸入治療,兩組均治療7 d,分別于治療前后檢測兩組肺功能、PCT、CRP表達水平。結(jié)果治療前兩組1 s用力呼氣容積(FEV1)、用力肺活量(FVC)比較差異無統(tǒng)計學(xué)意義(P0.05),治療后布地奈德組均高于對照組(P0.05)。治療前兩組PCT、CRP水平比較差異無統(tǒng)計學(xué)意義(P0.05),治療后布地奈德組均低于對照組(P0.05)。布地奈德組治療總有效率高于對照組(P0.05)。治療前兩組生活質(zhì)量綜合評定問卷-74項(GQOL-74)總評分比較無統(tǒng)計學(xué)差異(P0.05),治療后布地奈德組高于對照組(P0.05)。結(jié)論在常規(guī)治療的基礎(chǔ)上給予老年COPD急性加重期患者布地奈德霧化吸入治療能夠有效減輕患者體內(nèi)炎性反應(yīng),改善患者肺功能,療效確切,有利于提高患者生活質(zhì)量。
[Abstract]:Objective to investigate the efficacy of budesonide atomization inhalation in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) and its effects on pulmonary function and the levels of procalcitonin (PCT) and C-reactive protein (CRP). Methods two hundred and thirty elderly patients with acute exacerbation of COPD were randomly divided into budesonide group (n = 117) and control group (n = 113). On the basis of routine treatment, the control group was treated with methylprednisolone sodium succinate, while the control group was treated with budesonide atomized inhalation. Both groups were treated for 7 days, and the expression of PCT CRP was detected before and after treatment. Results there was no significant difference in forced expiratory volume (FEV1) and forced vital capacity (FVC) between the two groups before treatment (P0.05). After treatment, budesonide group was higher than control group (P0.05). There was no significant difference in PCT,CRP level between the two groups before treatment (P0.05). After treatment, budesonide group was lower than the control group (P0.05). The total effective rate of budesonide group was higher than that of control group (P0.05). Before treatment, there was no significant difference in the total score of GQOL-74 between the two groups (P0.05). After treatment, budesonide group was higher than the control group (P0.05). Conclusion on the basis of routine treatment, budesonide atomization inhalation in elderly patients with acute exacerbation of COPD can effectively reduce the inflammatory reaction and improve the pulmonary function of the patients. It is effective and beneficial to improve the quality of life of patients with acute exacerbation of COPD.
