【摘要】：目的：觀察二陳湯加味對COPD急性加重期(痰濁阻肺證)患者的臨床癥狀及肺功能的影響,客觀評估其臨床療效。方法：選取COPD急性加重期患者60例,西醫(yī)嚴(yán)重程度分級為Ⅰ～Ⅲ級,同時中醫(yī)辨證為痰濁阻肺證,以隨機分配方式分成治療組及對照組各30例。對照組給予單純西醫(yī)治療,治療組在常規(guī)西醫(yī)治療基礎(chǔ)上聯(lián)合二陳湯加味,療程共為14天,觀察患者治療前后肺功能、臨床癥狀、體征的變化。結(jié)果：1、二陳湯加味治療COPD急性加重期(痰濁阻肺證)的患者可明顯改善患者的臨床癥狀,治療組總有效率96.67%,對照組總有效率83.33%,兩者比較有統(tǒng)計學(xué)意義(P0.05),治療組的療效優(yōu)于對照組。治療組在主癥胸滿、咳嗽、痰多、色白黏膩或呈泡沫、短氣、喘息及兼次癥脘痞、納少、倦怠乏力等方面改善明顯優(yōu)于對照組,差異有統(tǒng)計學(xué)意義(P0.05)；兩組在兼次癥畏風(fēng)易汗、泛惡、便溏方面的改善無明顯差異(P0.05)。2、兩組患者肺功能指標(biāo)FEV1%pred、FEV1/FCV%較治療前均有所改善(P0.05)；且治療組在治療后FEV1%pred、FEV1/FCV%改善方面優(yōu)于對照組,有統(tǒng)計學(xué)意義(P0.05)。3、治療后,兩組患者血細(xì)胞分析、尿液常規(guī)分析、便常規(guī)、肝功能、腎功能、心電圖等安全性指標(biāo)無異常,且患者均未出現(xiàn)明顯不適反應(yīng)。結(jié)論：1、二陳湯加味可明顯改善COPD急性加重期(痰濁阻肺證)患者的臨床癥狀；2、二陳湯加味可改善COPD急性加重期(痰濁阻肺證)患者肺通氣功能；3、二陳湯加味治療COPD急性加重期(痰濁阻肺證)患者的治療過程中無明顯毒副作用及不良反應(yīng)。
[Abstract]:Objective: to observe the effect of Erchen decoction on the clinical symptoms and pulmonary function of patients with acute exacerbation of COPD. Methods: 60 patients with acute exacerbation of COPD were divided into treatment group (n = 30) and control group (n = 30). The control group was treated with simple western medicine, and the treatment group was treated with Erchen decoction on the basis of routine western medicine. The course of treatment was 14 days. The changes of pulmonary function, clinical symptoms and signs were observed before and after treatment. Results the clinical symptoms of the patients with acute exacerbation of COPD (phlegm turbid obstruction of lung syndrome) could be improved obviously by adding Wei 1 and Erchen Tang. The total effective rate of the treatment group was 96.677.The total effective rate of the control group was 83.33.The comparison between the two groups was statistically significant (P0.05). The curative effect of the treatment group was better than that of the control group. The treatment group was significantly better than the control group in chest filling, cough, phlegm, color, white, sticky or foam, shortness of breath, wheezing and chronic epigastric distension, less intake and fatigue, etc. The difference was statistically significant (P0.05). There was no significant difference in the improvement of loose stools (P0.05). The pulmonary function index (FEV1%pred,FEV1/FCV%) was improved in the two groups (P0.05), and the improvement of FEV1%pred,FEV1/FCV% in the treatment group was better than that in the control group (P0.05) .3.After the treatment, the two groups had blood cell analysis. The routine analysis of urine showed that there were no abnormal safety indexes such as stool routine, liver function, renal function and electrocardiogram, and no obvious discomfort was found in the patients. Conclusion the clinical symptoms of patients with acute exacerbation of COPD (phlegm turbid obstruction of lung syndrome) can be obviously improved by adding Wei 1 and er Chen Tang, and the pulmonary ventilation function of patients with acute exacerbation of COPD (phlegm turbid obstruction of lung) can be improved by Erchen decoction. There were no obvious side effects and side effects during the treatment of acute exacerbation of COPD (phlegm turbid obstruction of lung syndrome).
