【摘要】：目的：客觀評價清肺解毒扶正湯對老年肺炎患者的臨床療效及對C3、免疫球蛋白的影響，并對其安全性加以評估。 方法：老年肺炎（氣陰兩虛兼痰熱壅肺型）共納入72例。用隨機(jī)數(shù)字表法，分為治療組和對照組各36例，觀察過程中剔除脫落6例，最終治療組34例，對照組32例。兩組均采取平行對照設(shè)計，治療組在對照組綜合治療基礎(chǔ)上加服清肺解毒扶正湯，觀察兩組治療前后患者的臨床癥狀、體征、相關(guān)指標(biāo)，并對結(jié)果進(jìn)行統(tǒng)計分析。 結(jié)果：⑴兩組患者總療效、證候療效、證候積分比較，P＜0.05，說明治療組均優(yōu)于對照組；⑵治療組在治療咳嗽、痰量、發(fā)熱、出汗、乏力、納差、肺部體征的單個癥狀療效（P＜0.05）及療程方面明顯優(yōu)于對照組（P＜0.01）；⑶測定指標(biāo)：治療組治療后異常的C3、IgM、IgA與治療前比均降至正常范圍，與對照組比改善較為明顯（P＜0.01）。兩組IgG治療前后無顯著性差異（P＞0.05）；⑷治療組在胸片炎癥吸收、異常的白細(xì)胞改善以及痰培養(yǎng)方面優(yōu)于對照組（P＜0.05）；⑸安全性指標(biāo)和不良反應(yīng)觀察方面，差異無統(tǒng)計學(xué)意義（P＞0.05），兩組治療均相對安全。 結(jié)論：清肺解毒扶正湯不僅可以控制感染，促進(jìn)炎癥吸收，提高病菌清除率，，縮短療程，提高臨床療效，減少二重感染的發(fā)生，還可以調(diào)節(jié)免疫功能，提高患者免疫力，療效好于單純西藥治療。提示該藥具有良好的治療效果且相對安全，有重要的臨床應(yīng)用價值。
[Abstract]:Objective: to evaluate the clinical efficacy of Qingfei jiedu Fuzheng decoction in elderly patients with pneumonia and its effect on C _ 3 and immunoglobulin and to evaluate its safety. Methods: 72 cases of senile pneumonia (qi-yin deficiency and phlegm-heat obstructing lung type) were included. The patients were randomly divided into two groups: treatment group (n = 36) and control group (n = 36). In the course of observation, there were 6 cases of elimination and shedding, 34 cases of final treatment group and 32 cases of control group. The treatment group was treated with Qingfei jiedu Fuzheng decoction on the basis of comprehensive treatment in the control group. The clinical symptoms, signs and related indexes were observed before and after treatment, and the results were statistically analyzed. Results the total curative effect and syndromes score of the two groups were lower than those of the control group (P < 0.05), which indicated that the treatment group was superior to the control group in the treatment of cough, sputum volume, fever, sweating, fatigue and anorexia. The curative effect of single symptom of lung sign (P < 0.05) and the course of treatment were significantly better than that of control group (P < 0.01). After treatment, the abnormal C _ 3 IgM _ 3 IgA in the treatment group was decreased to the normal range compared with the control group (P < 0.01), and the improvement was more obvious than that in the control group (P < 0.01). There was no significant difference between the two groups before and after IgG treatment (P > 0. 05). The treatment group was superior to the control group in terms of chest film inflammation absorption, abnormal leukocyte improvement and sputum culture (P < 0. 05). There was no significant difference between the two groups (P > 0.05). Conclusion: Qingfei jiedu Fuzheng decoction can not only control infection, promote inflammation absorption, improve bacterial clearance rate, shorten course of treatment, improve clinical efficacy, reduce the occurrence of double infection, but also regulate immune function and improve patients' immunity. The curative effect is better than that of western medicine alone. It suggests that the drug has good therapeutic effect and is relatively safe and has important clinical application value.
