宣肺通腑法在重癥肺炎集束化治療中的臨床觀察

【摘要】：目的:觀察宣肺通腑法在重癥肺炎集束化治療中的臨床療效,探討其在重癥肺炎治療中的臨床價值。方法:將62例重癥肺炎患者隨機(jī)分為對照組(n=30)和治療組(n=32)。對照組給予氧療、抗感染、營養(yǎng)支持及維持內(nèi)環(huán)境穩(wěn)定等集束化治療;治療組在集束化治療基礎(chǔ)上加用晁老通腑方湯劑口服,每天1劑,共5 d。比較兩組患者在治療前及治療后第3、5天血C-反應(yīng)蛋白、降鈣素原、血氣分析的變化,機(jī)械通氣時間及住ICU時間、因為重癥肺炎導(dǎo)致的死亡率,并進(jìn)行統(tǒng)計學(xué)分析。結(jié)果:兩組患者在治療后第3、5天血C-反應(yīng)蛋白、降鈣素原、血氣分析等方面比較差異均有統(tǒng)計學(xué)意義(P0.05);兩組機(jī)械通氣時間、住ICU時間及死亡率比較,治療組預(yù)后更佳(P0.05)。結(jié)論:宣肺通腑法在重癥肺炎集束化治療中能有效改善氧合,協(xié)助抗炎、抗感染作用,減少機(jī)械通氣時間及留住ICU時間,可以有效提高重癥肺炎的治療效果。
[Abstract]:Objective: to observe the clinical effect of Xuanfei Tongfu method in the cluster therapy of severe pneumonia and to explore its clinical value in the treatment of severe pneumonia. Methods: 62 patients with severe pneumonia were randomly divided into control group (n = 30) and treatment group (n = 32). The control group was treated with oxygen therapy, anti-infection, nutritional support and maintenance of stable internal environment, while the treatment group was treated with Chao Lao Tong Fu decoction orally, 1 dose per day for 5 days. The changes of serum C-reactive protein, procalcitonin, blood gas analysis, mechanical ventilation time and ICU duration were compared between the two groups before and after treatment on the 5th day after treatment, and the mortality caused by severe pneumonia was analyzed statistically. Results: there were significant differences in serum C-reactive protein, procalcitonin and blood gas analysis between the two groups on the 3rd day after treatment (P0.05), and the time of mechanical ventilation, the duration of ICU residence and the mortality of the two groups were better than those in the treatment group (P0.05). Conclusion: Xuanfei Tongfu method can effectively improve oxygenation, assist anti-inflammatory and anti-infective effects, reduce mechanical ventilation time and retain ICU time in the treatment of severe pneumonia, and can effectively improve the therapeutic effect of severe pneumonia.
