重癥監(jiān)護室呼吸機相關肺炎患者的病原菌特征與耐藥性及相關因素分析

【摘要】:目的探討重癥監(jiān)護室(ICU)呼吸機相關性肺炎(VAP)的病原菌特征與耐藥性及相關因素。方法選擇2015年3月-2017年3月期間醫(yī)院ICU收治的機械輔助性呼吸超過48h的患者380例,診斷患者是否發(fā)生VAP,統(tǒng)計分析VAP患者的病原菌分布特點、主要病原菌的耐藥性,采用SPSS 20.0軟件分析影響VAP發(fā)生的相關因素。結果 380例患者中,有159例患者發(fā)生VAP,占41.84%;159例感染患者中共分離出263株病原菌,以革蘭陰性菌為主,共179株占68.06%;鮑氏不動桿菌對哌拉西林耐藥率最高;單因素分析結果顯示,年齡≥60歲、合并糖尿病、昏迷、機械通氣時間5d、住院時間14d、使用糖皮質激素、留置胃管、抗菌藥物使用種類3種能夠顯著影響VAP的發(fā)生(P0.05)。結論 VAP病原菌以革蘭陰性菌為主,合理選擇抗菌藥物并針對相關因素加強管理,控制VAP發(fā)生率。

Methods from March 2015 to March 2017, 380 patients with mechanical assisted respiration more than 48 hours from March 2015 to March 2017 were selected to diagnose the occurrence of VAP. The distribution characteristics of pathogens and drug resistance of main pathogens in VAP patients were statistically analyzed.
