【摘要】：目的:分析呼吸機(jī)相關(guān)性氣管支氣管炎對(duì)機(jī)械通氣患者臨床預(yù)后的影響。方法:對(duì)四會(huì)市人民醫(yī)院2015年10月至2017年2月收治的88例呼吸機(jī)相關(guān)性氣管支氣管炎患者予以分組研究,根據(jù)是否繼發(fā)呼吸機(jī)相關(guān)性肺炎(VAP)分為兩組,即對(duì)照組(n=53)、VAP組(n=35),對(duì)兩組患者治療時(shí)抗生素使用情況及治療后機(jī)械通氣時(shí)間與住院時(shí)間予以統(tǒng)計(jì)比較。結(jié)果:VAP組患者混合感染率為31.4%,多重耐藥菌分離率為82.9%,均高于對(duì)照組患者的13.2%、50.9%,組間比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。VAP組患者抗生素合理使用率為45.7%,低于對(duì)照組患者的92.5%,組間比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。VAP組患者機(jī)械通氣時(shí)間為(13.5±3.8)d,住院時(shí)間為(16.9±5.1)d,均長(zhǎng)于對(duì)照組患者的(9.3±3.6)d、(11.3±5.2)d,組間比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:在機(jī)械通氣中,早期給予合理抗生素治療,可有效控制呼吸機(jī)相關(guān)性氣管支氣管炎的進(jìn)展,對(duì)預(yù)防VAP有著積極的意義。
[Abstract]:Objective: to analyze the effect of ventilator associated bronchitis on the prognosis of patients with mechanical ventilation. Methods: 88 patients with ventilator-associated bronchitis treated in Sihui people's Hospital from October 2015 to February 2017 were divided into two groups according to whether they had secondary ventilator-associated pneumonia (VAP). The patients in the control group (n / 53) and VAP group (n / 35) were treated with antibiotics and the time of mechanical ventilation and hospitalization were compared statistically. Results the mixed infection rate and multidrug resistant bacteria isolation rate in the VAP group were 31.4 and 82.9, respectively, which were higher than those in the control group. The difference was statistically significant (P0.05). The rational use rate of antibiotics in the VAP group was 45.7, which was lower than that in the control group (92.5%). The difference was statistically significant (P0.05). The time of mechanical ventilation was (13.5 鹵3.8) days, the hospitalization time was (16.9 鹵5.1) days, which was longer than that of the control group (9.3 鹵3.6) d, (11.3 鹵5.2 days). The difference was statistically significant (P0.05). Conclusion: early treatment with antibiotics in mechanical ventilation can effectively control the progress of ventilator-associated tracheal bronchitis and has positive significance for the prevention of VAP.
