【摘要】：目的分析不同通氣障礙類型和不同級(jí)別慢性阻塞性肺疾病(COPD)患者抑郁情緒的出現(xiàn)情況。方法用美國(guó)流行病學(xué)研究中心抑郁量表(CES-D)對(duì)穩(wěn)定期患者進(jìn)行問(wèn)卷調(diào)查,所得肺功能結(jié)果先按呼吸專業(yè)評(píng)判標(biāo)準(zhǔn)將患者分為阻塞性通氣障礙和混合性通氣障礙兩組,再按GOLD分級(jí)標(biāo)準(zhǔn)將兩組患者各分為四組分別計(jì)算各組各級(jí)患者出現(xiàn)抑郁情緒者的百分率。結(jié)果從整體分級(jí)情況看,COPD患者抑郁情緒發(fā)生百分率與級(jí)差和患病時(shí)間均無(wú)趨同性。結(jié)論病變嚴(yán)重程度和患病時(shí)間的長(zhǎng)短只是COPD患者抑郁情緒發(fā)生率的影響因素之一。該文發(fā)現(xiàn)并首次報(bào)告,阻塞型通氣障礙的COPD患者未見(jiàn)有Ⅳ級(jí)的極重度患者,混合型通氣障礙的COPD患者未見(jiàn)有Ⅰ級(jí)的輕度患者,這一現(xiàn)象是我國(guó)日后重新修訂COPD診斷分級(jí)標(biāo)準(zhǔn)時(shí)必須加以關(guān)注和審視的一個(gè)問(wèn)題。
[Abstract]:Objective to analyze the occurrence of depression in patients with different ventilation disorders and different grades of chronic obstructive pulmonary disease (COPD). Methods the depression scale CES-Dof the American Epidemiological Research Center was used to investigate stable patients. The pulmonary function results were divided into obstructive ventilation disorder group and mixed ventilation disorder group according to the criteria of respiratory professional evaluation. According to gold classification, the two groups were divided into four groups to calculate the percentage of depression in each group. Results according to the overall grading, the incidence of depression, the difference of depression and the duration of disease were not similar in COPD patients. Conclusion the severity of the disease and the duration of the disease are only one of the factors affecting the incidence of depression in COPD patients.
