【摘要】：目的:分析不同類型特發(fā)性間質(zhì)性肺炎(IIP)患者膠原與血管生成相關(guān)因子的表達差異。方法:收集2003年1月至2014年11月間,北京朝陽醫(yī)院呼吸與危重癥醫(yī)學科住院確診的187例IIP患者,分為特發(fā)性肺纖維化118例(IPF組),除IPF外的其他類型IIP 69例(nIPF-IIP組)兩個亞組,其中nIPF-IIP組包括NSIP 30例,COP 29例;RB-ILD 10例。對照組68例,為男38例,女30例。收集患者及對照組支氣管肺泡灌洗液(BALF)和血清,應(yīng)用酶聯(lián)免疫吸附法(ELISA)分別檢測BALF及血清中I型前膠原(PCI)、Ⅲ型前膠原(PCⅢ)、人上皮細胞衍生中性粒細胞激活肽-78(ENA-78)和人γ干擾素誘導的蛋白-10(IP-10)的含量,對所測指標進行組間比較。結(jié)果:IPF患者組及nIPF-IIP組的BALF和血清中PCⅢ含量(ng/mL)均顯著高于對照組[BALF:4.65±0.06,2.90±0.40 vs.1.31±0.16,均P0.05;血清:11.49±0.98,3.37±0.16vs.1.98±0.09,均P0.05)在IPF患者組升高尤為明顯。而3組間BALF和血清中PC I含量差異均無統(tǒng)計學意義(均P0.05)。BALF中ENA-78含量(ng/mL)在IPF組和nIPF-IIP組均較正常對照組升高(17.85±0.44,18.89±0.45 vs.14.86±0.63,均P0.05;),但在二組之間無明顯差異(P=0.139)。而BALF中IP-10含量(ng/mL)在IPF組和nIPF-IIP組均較對照組降低(0.25±0.01,0.21±0.01 vs.0.35±0.03,均P0.05),且在nIPF-IIP患者的降低幅度較IPF患者大(P=0.040)。BALF中ENA-78含量與IP-10含量負相關(guān)(r=-0.378,P=0.02)。而血清ENA-78及IP-10變化不明顯。結(jié)論:IPF患者較nIPF-IIP患者存在更明顯的膠原代謝異常;IPF和nIPF-IIP患者肺臟的血管生成活性均增加。
[Abstract]:Aim: to analyze the expression of collagen and angiogenic factors in patients with different types of idiopathic interstitial pneumonia (IIP). Methods: from January 2003 to November 2014, 187 IIP patients who were admitted to the Department of Respiratory and critical Care Medicine, Chaoyang Hospital, Beijing, were divided into two subgroups: 118 patients with idiopathic pulmonary fibrosis (IPF) and 69 patients with IIP other than IPF (n = 69).
