瑞芬太尼對兔油酸性急性肺損傷的影響

【摘要】:目的:探討瑞芬太尼對兔油酸性急性肺損傷(ALI)的影響。方法:選取健康成年雄性新西蘭大白兔18只,由東南大學醫(yī)學院實驗動物中心提供,體質量2.5~3.5 kg,隨機分為3組(n=6),即對照組、油酸組和瑞芬太尼組。對照組經30 min靜脈輸注生理鹽水10 ml;油酸組經30 min靜脈注射油酸0.1 ml·kg-1;瑞芬太尼組靜脈輸注瑞芬太尼0.2μg·kg-1·min-1至處死,輸注15 min時開始給予油酸,方法同油酸組。各組大白兔分別在輸入瑞芬太尼或生理鹽水(以下簡稱給藥)前和給藥后6 h記錄平均動脈血氧分壓(Pa O2),及ELISA法檢測血漿細胞黏附分子1(ICAM-1)、TNF-α及IL-8濃度。給藥6 h后處死動物并迅速剖胸取肺臟,肉眼觀察肺組織病理改變,留取肺組織標本行電鏡觀察肺組織超微結構變化和測定濕干質量比。結果:瑞芬太尼組肺組織損傷程度逐漸減輕,明顯輕于油酸組,瑞芬太尼能顯著降低油酸性ALI兔血漿中ICAM-1、TNF-α及IL-8水平,改善ALI兔肺水腫,從而使PCO2和Pa O2均明顯改善。結論:瑞芬太尼能有效地抑制血管內皮細胞及肺泡上皮細胞的損傷和炎癥細胞浸潤,阻滯ALI時失控的炎癥反應,其機制可能與抑制ICAM-1、TNF-α及IL-8的表達有關。
[Abstract]:Objective: To investigate the effect of remifentanil on acute lung injury (ALI) in rabbits. Methods: 18 healthy adult male New Zealand white rabbits were selected from the experimental animal center of the Medical College of Southeast University. The body mass 2.5~3.5 kg was randomly divided into 3 groups (n=6), that is, the control group, the oleic acid group and remifentanil group. The control group was injected with 30 min intravenous infusion of saline. 10 ml; oleic acid group was injected with oleic acid 0.1 ml. Kg-1 via 30 min; remifentanil infusion was injected with remifentanil 0.2 mu g. Min-1 to death. When infusion 15 min, oleic acid was given to oleic acid group. The average arterial oxygen fraction was recorded before and after input of remifentanil or physiological saline (below a simple dose) and 6 h after administration. Pressure (Pa O2), and ELISA method were used to detect plasma cell adhesion molecule 1 (ICAM-1), TNF- alpha and IL-8 concentration. After 6 h, the animals were killed and the lungs were quickly removed. The pathological changes of lung tissue were observed by the naked eye. The ultrastructural changes of lung tissue and the ratio of wet dry mass were observed by the microscopic examination of lung tissue. Results: the degree of lung tissue injury in remifentanil group was the degree of lung tissue injury. Gradually reducing, obviously lighter than oleic acid group, remifentanil can significantly reduce the levels of ICAM-1, TNF- A and IL-8 in oleic acid ALI rabbit plasma, improve the pulmonary edema in ALI rabbit, thus improve both PCO2 and Pa O2. Conclusion: remifentanil can effectively inhibit the damage of vascular endothelial cells and alveolar epithelial cells and inflammatory cell infiltration, and prevent ALI out of control when ALI. The mechanism of inflammation may be related to the inhibition of the expression of ICAM-1, TNF- and IL-8.
