【摘要】：目的探討血脂在自身免疫性肺泡蛋白沉積癥(APAP)診斷與監(jiān)測中的意義。方法連續(xù)收集APAP患者血清脂代謝資料,分析32例非糖尿病APAP患者及100例健康體檢者血脂水平。用ELISA法測定APAP患者血清抗粒細胞巨噬細胞集落刺激因子(GM-CSF)抗體水平。分析血脂與乳酸脫氫酶、癌胚抗原、抗GM-CSF抗體、肺功能及血氣指標的相關性。結果 APAP組血清總膽固醇及低密度脂蛋白-膽固醇水平分別為(5.54±0.99)及(3.73±0.83)mmol/L,顯著高于健康對照組的(5.05±0.97)及(3.17±0.89)mmol/L(P均0.05);高密度脂蛋白-膽固醇顯著低于健康對照組[(1.10±0.18)比1.22(1.05-1.41)mmol/L,P均0.05]。APAP組的低密度脂蛋白/高密度脂蛋白及總膽固醇/高密度脂蛋白比值分別為3.47±0.90及5.14±1.12,顯著高于健康對照組的2.63±0.87及4.18±1.12(P均0.05)。APAP組的甘油三酯水平與健康對照組的差異無統(tǒng)計學意義(P0.05)。高密度脂蛋白-膽固醇與肺泡動脈氧分壓差呈負相關(r=-0.436,P0.05),與動脈血氧飽和度呈正相關(r=0.459,P0.05)。血脂指標與抗GM-CSF抗體無線性相關關系(P均0.05)。結論APAP患者存在血脂代謝異常,血脂異常與疾病嚴重程度有一定相關性,在APAP患者的評估與治療過程中需關注血脂異常。
[Abstract]:Objective to investigate the significance of serum lipids in the diagnosis and monitoring of autoimmune pulmonary alveolar proteinosis (APAP). Methods Serum lipid metabolism data of APAP patients were collected and blood lipid levels were analyzed in 32 patients with non diabetic APAP and 100 healthy controls. Serum anti-granulocyte macrophage colony stimulating factor (GM-CSF) antibody levels in patients with APAP were measured by ELISA method. To analyze the correlation between blood lipid and lactate dehydrogenase, carcinoembryonic antigen, anti GM-CSF antibody, pulmonary function and blood gas index. Results the levels of serum total cholesterol and low density lipoprotein cholesterol in APAP group were 5.54 鹵0.99) and 3.73 鹵0.83 mmol / L, respectively, which were significantly higher than those in healthy control group (5.05 鹵0.97) and 3.17 鹵0.89)mmol/L(P, respectively, and high density lipoprotein cholesterol was significantly lower than that in healthy control group (1.10 鹵0.18) vs 1.221.05-1.41mmolL / L, respectively. The ratio of low density lipoprotein / high density lipoprotein and total cholesterol / high density lipoprotein were 3.47 鹵0.90 and 5.14 鹵1.12, respectively, which were significantly higher than those of the healthy control group (2.63 鹵0.87 and 4.18 鹵1.12p). There was no significant difference in triglyceride levels between the 0.05).APAP group and the healthy control group (P 0.05). There was a negative correlation between high density lipoprotein cholesterol and alveolar arterial oxygen partial pressure difference (P0. 05) and a positive correlation between HDL-C and arterial oxygen saturation (P 0. 05). The correlation between serum lipids and anti-GM-CSF antibody was 0. 05% (P < 0. 05). Conclusion there is abnormal metabolism of blood lipids in patients with APAP, and there is a certain correlation between dyslipidemia and severity of the disease. Attention should be paid to dyslipidemia in the course of evaluation and treatment of patients with APAP.
