【摘要】：目的觀(guān)察沙美特羅替卡松口腔吸入治療慢性阻塞性肺疾病(COPD)合并肺動(dòng)脈高壓(PH)的臨床效果。方法選取COPD合并PH患者64例,隨機(jī)分為觀(guān)察組及對(duì)照組各32例。對(duì)照組給予化痰、止咳、吸氧、抗感染等常規(guī)治療,觀(guān)察組在常規(guī)治療基礎(chǔ)上給予沙美特羅替卡松粉劑(沙美特羅:丙酸氟替卡松為50μg:500μg)口腔吸入,1吸/次,2次/d。兩組均治療6個(gè)月。對(duì)比兩組療效,治療前后肺通氣功能、平均肺動(dòng)脈壓(m PAP),血清低氧誘導(dǎo)因子1α(HIF-1α)、Apelin水平、生活質(zhì)量評(píng)分。結(jié)果觀(guān)察組總有效率(90.6%)與對(duì)照組(62.5%)比較,P0.05。治療后觀(guān)察組第1秒用力呼氣容積、用力肺活量、第1秒用力呼氣容積占預(yù)計(jì)值百分比、m PAP、血清HIF-1α水平、血清Apelin水平、生活質(zhì)量評(píng)分與對(duì)照組比較,P均0.05。結(jié)論沙美特羅替卡松口腔吸入治療COPD合并PH療效佳,可有效改善患者肺功能,降低m PAP,改善其生活質(zhì)量。
[Abstract]:Objective to observe the clinical effect of salmeterol fluticasone inhalation in the treatment of chronic obstructive pulmonary disease (COPD) with pulmonary hypertension (PH). Methods 64 cases of COPD complicated with PH were randomly divided into two groups: observation group (n = 32) and control group (n = 32). The control group was given routine therapy such as phlegm reduction, cough relief, oxygen inhalation, anti-infection and so on. The observation group was given salmeterol: fluticasone propionate 50 渭 g / 500 渭 g oral inhaled oral cavity for 1 / 2 / d on the basis of routine treatment. The control group received oral inhalation of salmeterol: fluticasone propionate (50 渭 g / 500 渭 g). Both groups were treated for 6 months. The therapeutic effect, pulmonary ventilation function, mean pulmonary artery pressure (PAP), serum level of hypoxia inducible factor-1 偽 (HIF-1 偽) and quality of life (QOL) were compared between the two groups. Results the total effective rate of the observation group was 90.6%, compared with that of the control group (62.5%). After treatment, forced expiratory volume, forced vital capacity, forced expiratory volume in the first second and the percentage of forced expiratory volume to predicted value in the observation group were 0. 05%, serum HIF-1 偽 level, serum Apelin level, quality of life score were 0. 05% compared with the control group (P < 0. 05). Conclusion salmeterol fluticasone inhalation is effective in the treatment of COPD with PH, which can effectively improve pulmonary function, decrease Mapp and improve the quality of life.
