【摘要】：目的:觀察支炎合劑對熱毒閉肺型支原體肺炎治療的臨床療效,并初步探討其作用機制。方法:將符合納入標準的60例患者隨機分為支炎合劑組、阿奇霉素組,每組各30例。支炎合劑組予支炎合劑+基礎(chǔ)治療;阿奇霉素組予阿奇霉素+基礎(chǔ)治療。兩組療程均為2周。治療結(jié)束后對疾病療效、癥狀積分、MP-Ab、胸部X線、WBC、不良反應(yīng)等方面進行統(tǒng)計及分析,來判定兩組治療熱毒閉肺型支原體肺炎的療效及安全性。結(jié)果:1.兩組癥狀積分治療前后差異有顯著統(tǒng)計學(xué)意義(P0.01),治療后組間對比,差異有統(tǒng)計學(xué)意義(P0.05)。2.MP-Ab抗體滴度:治療后兩組患者MP-Ab均有升高趨勢。3.支原體肺炎患者WBC檢查多在正常范圍內(nèi)。兩組患者治療前后CRP比較差異有統(tǒng)計學(xué)意義(P0.05),治療后組間CRP比較差異無統(tǒng)計學(xué)意義(P0.05)。4.兩組胸部X線治療前后比較,差異有統(tǒng)計學(xué)意義(P0.05);治療后組間比較,差異無統(tǒng)計學(xué)意義(P0.05)。5.阿奇霉素組總有效率為85.7%,支炎合劑組總有效率為90%,支炎合劑組療效優(yōu)于阿奇霉素組,兩組療效差異具有統(tǒng)計學(xué)意義(P0.05)。6.不良反應(yīng):阿奇霉素組有6例出現(xiàn)不同程度納食減少、惡心、腹脹、腹瀉等胃腸道反應(yīng)。支炎合劑組未出現(xiàn)明顯不良反應(yīng)。結(jié)論:支炎合劑可有效治療熱毒閉肺型支原體肺炎患者,且不良反應(yīng)少,值得臨床進一步推廣使用。
[Abstract]:Objective: to observe the clinical effect of Zhiyan mixture in treating mycoplasma pneumoniae pneumonia caused by heat toxin. Methods: 60 patients who met the inclusion criteria were randomly divided into Zhiyan mixture group and azithromycin group with 30 cases in each group. Zhiyan mixture group was treated with Zhi Yan mixture and azithromycin group with azithromycin. The course of treatment was 2 weeks in both groups. In order to evaluate the efficacy and safety of the two groups in treating mycoplasma pneumoniae pneumonia caused by heat toxin, the curative effect, symptom score, chest X-ray and side effects of MP-Abb, chest X-ray, and adverse reactions were analyzed and analyzed after the treatment. The result is 1: 1. The difference between the two groups before and after treatment was statistically significant (P 0.01), and the difference was statistically significant after treatment. 2. The antibody titer of MP-Ab: after treatment, the MP-Ab of the two groups were all increased. 3. The WBC examination of mycoplasma pneumonia patients was mostly within normal range. There was significant difference in CRP between the two groups before and after treatment (P 0.05), but there was no significant difference in CRP between the two groups after treatment. There was significant difference between the two groups before and after chest X-ray treatment, but there was no significant difference between the two groups after treatment. The total effective rate of azithromycin group was 85.7 and that of Zhiyan mixture group was 90. The curative effect of Zhiyan mixture group was better than that of azithromycin group. The difference between the two groups was statistically significant. Adverse reactions: in azithromycin group, there were 6 cases of gastrointestinal reactions, such as decrease of food intake, nausea, abdominal distension, diarrhea and so on. There was no obvious adverse reaction in the group of Zhi Yan mixture. Conclusion: Zhiyan mixture can effectively treat the patients with heat-toxic mycoplasma pneumoniae pneumonia, and the adverse reactions are few, which is worthy of further clinical application.
