【摘要】：目的觀察錫冶煉工塵肺病患者胸部影像學改變和轉歸情況。方法對10例錫冶煉工塵肺病病例進行胸部X射線和電子計算機斷層掃描(CT)攝影及動態(tài)觀察,分析其肺部影像學特點和轉歸情況。結果 10例錫冶煉工塵肺病患者的臨床癥狀輕微,未見合并肺結核,肺通氣功能改變不明顯。診斷時X射線高千伏胸片結果顯示:兩肺小陰影形態(tài)以圓形為主,多分布在雙肺中上肺區(qū);多數(shù)病例肺紋理扭曲、變形或增多、模糊,肺門影增大增粗,淋巴結點狀鈣化,個別狀如蛋殼,未見胸膜改變和肺氣腫改變;5~10年的動態(tài)觀察除1例患者小陰影增多外,其余均未見明顯改變。2011年胸部CT檢查可見2~5 mm的結節(jié)狀影;其中,7例患者小結節(jié)從兩上肺葉均勻分布至中葉、兩肺下葉背段,下葉基底段病變減少;病變呈彌漫分布全肺葉,且胸膜下有小結節(jié)者3例;小葉間隔不同程度增厚5例;有小葉中心型或間隔旁肺氣腫4例;其中1例有肺大泡形成。10例患者2016年胸部CT檢查結果與2011年比較,彌漫性肺結節(jié)增多2例,小葉間隔增厚明顯3例,肺氣腫有進展2例。結論錫冶煉工塵肺病患者肺部小陰影經(jīng)5~10年的X射線動態(tài)觀察未見明顯吸收,且病變有進展。CT檢查有助于對錫冶煉工塵肺病進行追蹤觀察。
[Abstract]:Objective to observe the imaging changes and prognosis of pneumoconiosis in tin smelting workers. Methods 10 cases of pneumoconiosis of tin smelting workers were studied by chest X-ray and computed tomography (CT). The imaging features and prognosis of the lung were analyzed. Results the clinical symptoms of 10 patients with tin smelter pneumoconiosis were mild, no pulmonary tuberculosis was found, and the pulmonary ventilation function was not obvious. The results of X-ray high kilovolt chest radiographs showed that the small shadows of the two lungs were mainly circular in shape, mostly distributed in the upper and middle lung areas of both lungs, and in most cases, the lung was distorted, distorted or increased, blurred, the hilar shadow enlarged and thickened, and the lymph nodes were dotted with calcification. There were no pleural changes and emphysema changes in 5 ~ 10 years. Except for one patient with increased small shadow, there were no obvious changes in the other cases. In 2011, 2 ~ 5 mm nodular shadow could be seen on chest CT examination. In 7 cases, the nodules were evenly distributed from the upper lobes to the middle lobes, the dorsal segments of the lower lobes decreased, the lesions distributed diffusely throughout the lungs and there were small nodules under the pleura in 3 cases, and the interlobular septum thickened in 5 cases. There were 4 cases of central lobular emphysema or paracentricular emphysema, of which 1 case had alveolus formation and 10 cases had pulmonary bullae formation. Compared with 2011, 2 cases had diffuse pulmonary nodules, 3 cases had thickening interlobular septum, and 2 cases had emphysema. Conclusion the small shadow of lung in patients with pneumoconiosis of tin smelting workers has not been absorbed obviously by X-ray dynamic observation for 5 ~ 10 years, and CT examination is helpful for tracing and observation of pneumoconiosis in tin smelters.
